A pilot test of a text message-based transgender and nonbinary affirmative cognitive-behavioral intervention for anxiety and depression.

Abstract: Transgender and nonbinary (TNB) people report experiencing elevated rates of discrimination and psychological distress (James et al., 2016). Transgender-affirming interventions are recommended to address psychological distress (e.g., anxiety, depression) reported by TNB individuals (Craig & Austin, 2016). TNB people in rural areas report barriers (e.g., lack of financial resources, long commutes) to accessing affirmative and/or quality treatment (Knutson, Koch, Arthur, Mitchell, & Martyr, 2017). Electronically… Show more

“…Depressive symptoms were significantly lower in the intervention group at the 2-week follow-up. This finding is consistent with the effects of a cognitive behavioral therapy based coping skills intervention for sexual and gender minority youth (Craig & Austin, 2016;Knutson et al, 2021). The findings from the present study suggest that an emotion-focused intervention highlighting the positive identities and narratives of TNB people may be helpful in alleviating depressive symptoms.…”

“…Participants reported significant decreases in depression immediately after the retreat that persisted through the 3-month follow-up. Knutson et al (2021) conducted a 6-week long transgenderaffirmative text message intervention with 14 TNB participants over the age of 18. In the intervention, trained coaches with master's degree in counseling, clinical psychology, or counseling psychology led participants through a 6-week manualized protocol to reduce anxiety and depressive symptoms using transgender-affirming techniques.…”

“…Social support often shows the largest effect sizes in negatively predicting depression and anxiety 76 , 137 , 184 and suicide risk 185 187 among other predictors such as positive identity development 83 , 188 and use of facilitative coping strategies 21 , 76 . Several interventions have specifically targeted social support to improve mental health and well-being 189 , 190 . In one pilot study, an online intervention to assist parents in providing social support to their TNB children in the USA, the intervention was feasible and improved parental support.…”

“…Such approaches could be engaged to help transgender women understand and affirm their transgender identity and learn to recognize and cope with sexual and gender-based sources of stress, such as transphobia or cissexism (Austin et al, 2018). One such intervention administered among transgender and nonbinary adults demonstrated significant decreases in anxiety and depression among participants, as well as increases in experiences of social support (Knutson et al, 2021). These findings suggest the importance of the development of CBT-informed interventions to help transgender women identify and cope with the sources of violence they have experienced.…”
