A pilot randomized trial comparing symptomatic vs. hemoglobin‐level‐ driven red blood cell transfusions following hip fracture

Abstract: Symptomatic transfusion may be an effective blood-sparing protocol associated with the transfusion of appreciably fewer units of RBCs and lower mean hemoglobin levels than are associated with the threshold transfusion policy. However, it is unknown whether these two clinical strategies have comparable mortality, morbidity, or functional status. A definitive trial is needed.

“…Carson et al [72] Grover et al [73] Foss et al [74] Carson et al [75] Parker [76] So-Osman et al [77] All these RCTs focused on major lower limb surgery -elective hip/knee arthroplasty or hip fracture repair. There was heterogeneity between the studies with regard to the transfusion triggers used.…”

“…Carson et al found that the mortality risk increases in patients with Hgb levels lower than 11 g/dL [4]. However, in two smaller studies, there was no significant relationship between levels of Hgb and mortality [29,30]. This finding in these two studies may be a result of a lack of statistical power: both groups had an increased risk because they had some level of anemia.…”
