Amlodipine besylate belongs to a group of blockers of calcium channels, derivatives of 1,4-dihydropyridine, is used to treatment of arterial hypertension and vasospastic forms of angina pectoris. According to the literature sources, amlodipine in case of overdose can provoke the development of breast cancer, cause ischemia of the optic nerve. Deadly poisoning with amlodipine may accompany drug overdoses or suicidal cases. The development of highly sensitive and selective methods for the study of amlodipine during forensic toxicological examination of biological material is an actual task. One of the important stages of forensic toxicological examination is the choice of organs of corpses for investigation of them for the presence of poisons, which requires the analysis of amlodipine in the organs of poisoned animals. The aim of the investigation is the study of the distribution of amlodipine in organs of poisoned animals, using developed highly sensitive and selective techniques for the analysis of amlodipine in biological objects. Materials and methods. To study the distribution of amlodipine in organs of poisoned animals, rats weighing 200-250 g were used. Amlodipine besylate solution was administered to rats using a probe in the stomach from the calculation of 70,0 mg / kg, after 3 hours rats were decapitated. For the study blood, urine, heart, liver, kidneys, lungs, stomach and intestine with contents and spleen were taken. Control experiments were delivered with the appropriate organs in parallel. Extraction of the substance was carried out with a mixture of methanol and acetonitrile (2: 3), purification was carried out by extraction of impurities with diethyl ether at pH 2,0-3,0 and TLC-method. Amlodipine was identified in biogenic extracts by the TLC-method in three systems of mobile solvents. The substance content was determined by spectrophotometry methods in the UV region of the spectrum. Results and discussion. It is established that the highest amount of amlodipine was found in the stomach and intestine with the content, which is typical for acute poisoning. Less amount of amlodipine is found in the liver, kidneys and urine - organs and liquids that provide active detoxification of the body. According to the results of the research, it was found that in case of lethal poisoning, amlodipine for forensic toxicological studies should be carried out in urine, stomach with contents, intestine with contents, liver, spleen and kidneys. Conclusions. The distribution of amlodipine in organs of poisoned animals with oral administration was studied. It is established that in case of lethal poisoning with amlodipine, for forensic-toxicological research it is necessary to be conducted in urine, stomach with contents, intestine with contents, spleen and kidneys. The developed methods can be proposed for introduction into the practice of the Bureau of Forensic Medical Examination, toxicological centers

Comparison of Efficacies of Commonly Used Hypertension Treatment Modalities: A Retrospective Study of 1900 Participants in a Hypertension Clinic

Background:Although various antihypertensive medications are available, some hypertensive patients have uncontrolled blood pressures, especially in the clinic. The aim of the present study was to compare the efficacies of various antihypertensive therapies in our hypertension (HTN) clinic (monotherapy vs. combination therapy, fixed-dose combination (FDC) versus free equivalent combination (FEC), and diuretics versus non-diuretics. Material/Methods:In this retrospective study, patients at the HTN clinic of the Third Xiangya Hospital with primary hypertension were enrolled from June 2016 to February 2017. Data on participants' basic characteristics, blood pressure data, and treatment modalities were collected. The proportions of participants attaining target blood pressure after treatment with antihypertensive modalities were calculated and compared. Results:Among 1900 participants, combination therapy had a better control efficacy than monotherapy (P<0.0005).When HTN was treated by 2 kinds of drugs, FEC was used much more frequently than FDC (P<0.0005). In grade 3 HTN, FDC had a higher control rate (P=0.002). If more than 2 kinds of drugs were used, FDC+OTHER had a slightly higher control rate in grade 2 and 3 (42.1% vs. 38.5%, P=0.724; 36.2% vs. 31.0%, P=0.526, respectively). Therapies with diuretics had better control rates than those without diuretics (43.1% vs. 36.9%, P=0.025). Conclusions:In our clinic, FEC was prescribed more often than FDC. When blood pressure is significantly elevated, especially at levels 2 or 3, FDC seems to have a better control rate than FEC. Therapies with diuretics controlled HTN more efficiently.

Choice Between Free Combination of Antihypertensive Agents and Fixed Dosed Combinations in the Treatment of Arterial Hypertension

Most patients with arterial hypertension require more than one antihypertensive drug for blood pressure target achievement. Some patients are recommended for a multi-pill antihypertensive regimen, others — treatment with fixed dosed combinations in one tablet. Analysis of elibrary and PubMed publications in the period mostly from 2014 to 2024 concerning the choice of two-component combined antihypertensive agents containing renin-angiotensin system inhibitor and diuretic or calcium channel blocker, revealed that fixed-dose combinations (FDC) use and taking one tablet once a day improves adherence to treatment and facilitates blood pressure control. Although the cost of FDC containing the renin-angiotensin-aldosterone system inhibitor and a thiazide/thiazide-like diuretic or calcium channel blocker is in most cases higher than the same drugs taken separately, the use of fixed combinations, increasing patient adherence to therapy, has clinical advantage in terms of the effectiveness of lowering blood pressure, which confirms their economic feasibility. On the other hand, the use of free combination therapy in two different tablets, when taken separately during the day, can sometimes provide a more sustained antihypertensive effect over 24 hours. Evidence of the effectiveness of blood pressure control for FDCs is often extrapolated from data on free combinations. In addition, FDCs are characterized by less detection of possible ineffectiveness of one of the components. The range of FDCs and the dosage ratios of the components presented in them is gradually expanding, but the choice among free combinations is still wider. In addition, the list of vital and essential drugs (VED) for 2024 does not contain FDCs for antihypertensive drugs, which excludes the possibility of free receiving them on a preferential basis and gives the opportunity for the manufacturer to set prices for them. Despite the fact that recently the scientific community has recommended the use of FDC antihypertensive drugs as initial therapy due to better compliance with the regimen, and therefore clinical effectiveness and economic feasibility, it cannot be said that there is no space left for free combinations of antihypertensive drugs in the treatment of arterial hypertension. The choice of doctor, frequency of prescription, share of purchases of the FDCs in the Russian Federation, review of their consumption requires further analysis.
