A Patient with Eosinophilic Esophagitis and Herpes Simplex Esophagitis: A Case Report and Literature Review

Abstract: Acute herpes simplex esophagitis (HSE) is common in immunocompromised patients. Eosinophilic esophagitis (EoE) is characterized by immune-mediated eosinophil-predominant esophageal inflammation. We report a patient with human immunodeficiency virus infection who presented with dysphagia and odynophagia and was found to have HSE and EoE. The combination of these two relatively rare conditions suggests possible predisposition.

“…Only one serious AE (oesophageal herpes simplex virus [HSV]) was considered related to study drug, which resolved 21 days after onset following antiviral treatment. However, we note that studies have observed HSV infection in immunocompetent individuals with EoE, irrespective of topical corticosteroid use, raising doubts about this association 3,4 …”

“…However, we note that studies have observed HSV infection in immunocompetent individuals with EoE, irrespective of topical corticosteroid use, raising doubts about this association. 3,4 Although 14.4% and 4.8% of participants receiving BOS 2.0 mg b.i.d. reported adrenal AEs and abnormal adrenocorticotropic stimulation tests, respectively, most were not considered related to study drug, were mild to moderate in severity and were not associated with signs or symptoms of glucocorticoid excess or adrenal dysfunction.…”

“…Герпесвирусный эзофагит подтверждается при позитивном иммуногистохимическом окрашивании с HSV I+II, положительной ПЦР на HSV и положительных серологических тестах с определением IgG и IgM к HSV. В наблюдениях на небольших группах пациентов описана связь герпесвирусного эзофагита с эозинофилией слизистой оболочки пищевода и ЭоЭ как у иммунокомпетентных детей, так и у иммунокомпрометированных взрослых [46][47][48][49].
