A Nursing Clinical Decision Support System and Potential Predictors of Head-of-Bed Position for Patients Receiving Mechanical Ventilation

Abstract: Background Patients receiving mechanical ventilation are at high risk for pneumonia due to aspiration. Published guidelines recommend elevating the head of the bed 30º to 45º, if not contraindicated, to reduce risk, but this intervention is underused. Objectives To facilitate incorporating evidence-based practice by improving positioning of patients receiving mechanical ventilation and to identify patient and nurse characteristics that predict use of the guideline. Methods A modified interrupted time-series de… Show more

“…The use of IT in the field of healthcare was found to improve the healthcare system and increase in the quality of care (Alipour et al, 2012;Azizi & Abdolkhani, 2012;Diamond, Johnson, Padman, Zheng, & Payne, 2000;Jamal, McKenzie, & Clark, 2009;Karami, 2015). These improvements are evident in areas such as cardiac care, intensive care, prescription drugs, and nursing (Karami, 2015;Bertsche et al, 2010;Lyerla, LeRouge, Cooke, Turpin, & Wilson, 2010;Rollman et al, 2002;Walsh et al, 2010).…”

“…Although they saw a statistically significant improvement in the mean degree of elevation after the intervention, they also noted a discrepancy of 4 between the documented incline (as measured by the bed scale and recorded by the nursing staff) and the observed incline, as measured by the serial prevalence measurements performed by a member of the investigative team by using a protractor. 23 At Intermountain Healthcare, patients identified by a CDS tool as being at high risk for colonization with methicillin-resistant Staphylococcus aureus (MRSA) are flagged at the nursing station as needing a surveillance culture. These patients were 5 times more likely to be colonized than low-risk patients in the same medical unit, and results were available on average within 16 hours.…”

“…Furthermore, interface pressures were not recorded for angles greater than 45° because elevation of the head of the bed more than 45° is rarely used in current practice. [31][32][33] For the ProfiCare and TheraPulse ATP mattresses, alternating regional pressure changes and pulsation therapy, respectively, are offered as additional features and may have additional beneficial effects on tissue perfusion. Both features may temporarily and regionally influence interface pressure.…”
