Background: Temporomandibular disorders are musculoskeletal conditions characterized by facial pain and impaired temporomandibular joint function, limited mouth opening, joint and muscular pain, and noises during mandibular movements are some of the most common symptoms. The most frequent cause of temporomandibular joint dysfunction is internal derangement (ID), which refers to an alteration in the normal pathways of motion of the joint that largely involves the function of the articular disc, therefore, these alterations have been also referred to as disc derangement. Arthrocentesis is a minimally invasive technique, less expensive than surgical treatment. Adhesions are released after arthrocentesis of the upper joint space under sufficient hydraulic pressure. Intra-articular ozone gas injection is used as conservative treatment modalities for ID of the temporomandibular as it possesses anti-inflammatory, analgesic effects, enhancement the host defense mechanism and accelerates the healing process of the damaged cells. Aim: The aim of this study was to compare the effectiveness of ozonized water against lactated ringer solution in the arthrocentesis of the temporomandibular joint. Patients and Methods: Sixty patients were used in this study, suffered from ID of the temporomandibular joint treated by arthrocentesis under hydraulic pressure and were allocated into 2 groups; the study group (A), which included 30 patients, managed by arthrocentesis utilizing ozonized water and the control group (B) with 30 patients also treated by the same procedure using ringer lactate solution. Visual analog scale pain scores, temporomandibular joint sounds, and maximal mouth opening were assessed preoperatively and at different intervals postoperatively. Results: The age in this study ranged from 14 to 66 years. The mean age of group A was 29.93 years with an SD of ±11.79. For group B, the mean age was 27.56 years and the SD was ±10.80, the prominent percentage in both groups was <30 years. Regarding sex, 45 patients were females, whereas the males were 15 with a ratio of 3:1. Group A registered the highest reduction in the visual analog scale at all postoperative intervals. With respect to the mouth opening, there was no significant difference in maximal mouth opening between the 2 groups after 1 week and 12 weeks in comparison with the preoperative measurements. The temporomandibular joint sounds improved in all patients in group A after 12 weeks, whereas in group B the sounds dropped to 33.3%. Conclusion: The data from the present study suggested more favorable treatment outcomes for ozonized water lavage and it is a promising new treatment modality for the relief of symptoms associated with the ID of the temporomandibular joint.

Temporomandibular joint disorders (TMD) generate pain and difficulties for mouth opening affecting the patients’ quality of life. Ozone is an emerging therapy that has been proposed as a potential treatment, due to that, the evidence about its efficacy should be reviewed. Therefore, this work aimed to conduct a comprehensive systematic review to address the efficacy of ozone therapy for the treatment of pain and limited mouth opening in patients with TMD. The design of the included studies was clinical trials and observational studies, whereas, a series of cases, in vivo, and in vitro studies were excluded. The search was performed in PubMed, ClinicalTrials, Web of Science, and Scopus. Gray literature was searched at Google Scholar. Relevant data of all included studies were recorded. The risk of bias (using RoB 2) and the quality (using Grading of Recommendations Assessment, Development, and Evaluation) assessments were carried out. Meta-analyses using random-effects models of pain and maximal mouth opening data were performed. This review included 8 studies with 404 participants suffering limited function and pain related to TMD. At the overall bias of the studies, 25% exhibited some concerns and 75% had high risk; and the quality of the studies was low. The analysis of the included studies suggests that ozone therapy can diminish pain and improve the maximal mouth opening in TMD patients. However, there is no conclusive evidence of ozone therapy as a superior treatment for TMD compared with occlusal splint and pharmacotherapy.

O ozônio terapêutico é uma mistura de oxigênio (O2) e ozônio (O3) que pode ser produzida em geradores de ozônio e possui várias modalidades de administração, como tratamento minimamente invasivo na redução da dor muscular e no tratamento de pacientes com disfunção temporomandibular (DTM). O objetivo do estudo foi estudar a eficiência da ozonioterapia no tratamento das disfunções temporomandibulares. Tratou-se de uma revisão da literatura com abordagem narrativa, utilizando-se as bases de dados: Literatura Latino Americana e do Caribe em Ciências da Saúde (LILACS), Literatura Internacional em Ciências da Saúde (MEDLINE), US National Library of Medicine (PubMed) e Scientific Eletronic Library Online (SciELO), utilizando-se os descritores “síndrome da disfunção da articulação temporomandibular”, “ozônio” e “ozonioterapia”. Foram incluídos estudos clínicos, descritivos, quantitativos, qualitativos e relatos de casos completos que abordaram a ozonioterapia no tratamento das DTMs nos idiomas português, inglês e espanhol no período de 2012 a 2023, excluindo-se dissertações, teses, editoriais, estudos incompletos e os que não apresentaram relação com a temática pesquisada. Os estudos selecionados apresentaram resultados promissores e satisfatórios na terapêutica com ozônio, quer seja utilizado de forma isolada ou combinada com a terapia convencional. Embora exista a necessidade de mais pesquisas na literatura, a ozonioterapia demostra muitos benefícios, reduzindo sintomas de dor e fadiga associados a DTM e melhorando a função mandibular, sendo um método terapêutico minimamente invasivo e eficaz para tratamento de pacientes com DTM.
