A novel PillCam Crohn’s capsule score (Eliakim score) for quantification of mucosal inflammation in Crohn’s disease

Abstract: Introduction Capsule endoscopy is an important modality for monitoring of Crohn’s disease. Recently, a novel panenteric capsule, PillCam Crohn’s (Medtronic, USA), was approved for use. No quantitative index of inflammation for this method is currently available. This sub-study of a prospective randomized controlled Comprehensive individUalized pRoactive ThErapy of Crohn’s Disease trial (CURE-CD) which aimed to compare the correlation and reliability of the novel PillCam Crohn’s score with the existing small bo… Show more

“…It was compared to LS and a strong correlation (r = 0.8 for reader 1 and r = 0.82 for reader 2, P < 0.001 for both) was found between the scores. The calculation of the cut-off values is LS 135 = 4, LS 350 = 5, LS 790 = 8 102 .…”

“…Recent efforts to validate a scoring system for PCE are welcome. 29 Finally, clinicians may have chosen a panenteric examination because of a clinical suspicion of small bowel disease, thereby making this a non-representative population; were this the case, however, much of it was previously unidentified as the panenteric study still had a major impact on the diagnosis of L4 disease. Finally, assessment of small and large bowel was variable in terms of both completeness and proximity in time prior to PCE.…”

“… 70 Moreover, thanks to a new integrated reading system, this video capsule permitted the recording and further comparison of patients’ images over time, thus monitoring disease activity and extent under treatment. 71 It is particularly relevant in the pediatric population, where this type of “pan-endoscopy” was shown to effectively monitor mucosal inflammation and successfully guide a treat-to target strategy (twofold increase in the rate of MH and deep remission from baseline to week 54; p < 0.05). 72 …”
