A Novel Approach for Evaluating the Risk of Health Care Failure Modes

Abstract: Failure mode and effects analysis (FMEA) can be employed to reduce medical errors by identifying the risk ranking of the health care failure modes and taking priority action for safety improvement. The purpose of this paper is to propose a novel approach of data analysis. The approach is to integrate FMEA and a mathematical tool-Data envelopment analysis (DEA) with "slack-based measure" (SBM), in the field of data analysis. The risk indexes (severity, occurrence, and detection) of FMEA are viewed as multiple i… Show more

“…Failures are prioritized according to how serious their consequences are, how frequently they occur, and how easily they can be detected. The process differs from commonly utilized retrospective improvement methodologies, such as root cause analysis, which is used to investigate safety events after they occur (13)(14)(15). This methodology has been successfully applied in other health care arenas (16,17), but there is limited description of its use in evaluating anesthesia processes (18).…”

“…DEA is an analytical tool used for business effectiveness evaluations in many areas (Hadad, Friedman, & Israeli, 2005;Wang & Lu, 2015;Yao, Han, & Luo, 2010;Zhu, 2009). DEA is also known to be able to rank different models (Jablonksy, 2016) evaluate risks of the units surveyed (Aldamak & Zolfaghari, 2017;Chang, Chung, Sun, & Yang, 2012), and analytically rank risks (Sheludko & Kirova, 2018). Characteristic which makes it a very good tool for risk ranking is it's capable of handling both one input and one output parameters, and many parameters of one object.…”

“…Although the administrators originate from the Middle East and share the same religion and heritage, every country and region has its own unique culture, and understanding and coping with all these differences is time-consuming. Personal privacy and confidentiality issues are also serious problems that could discourage people from discussing their problems online [11,12]. Some people may expose their health issues that should not be discussed in public [13].…”
