A New, Wider-Diameter Crawford Tube for Stenting in the Lacrimal Drainage System

Abstract: The technology of using a metal ferrule at the junction of the standard Crawford probe and a larger silicone tube was successful in creating a Crawford system with greater dilation capability than the standard Crawford system. The technique of probing and intubation with this larger Crawford system may be a simple alternative to open revision of a failed dacryocystorhinostomy procedure.

“…Tables 5 and 6 summarize the tuberelated complications in the present study and the complications associated with previous double silicone stenting studies. Compared with the single-stent group, no significant complications appeared, which was consistent with previous reports [11][12][13][14][15]. The other treatment methods implicated for canalicular obstruction are DCR combined with membranectomy [5,6], trephination and silicone tube stenting [7,16], ballooning with/without silicone stenting [17,18], ballooning canaliculoplasty following lacrimal trephination [8], and intraoperative use of mitomycin C [9,10].…”

“…Hurwitz [11] suggested the use of a larger-diameter Crawford tube for stenting in the lacrimal drainage system in patients with failed DCR. In this report, a larger-diameter stent maintained a better separation of the lacrimal channel walls, which would be expected to decrease the chance of restenosis following stent removal.…”

“…Reports speculated that a larger-diameter stent would keep the lacrimal channel walls more separated. This would decrease the chance of restenosis after stent removal (Hurwitz 2004). Alternatively, a wider dilation of the lacrimal system may be achieved by double intubation of the stent (Mauffray et al 2004;Caversaccio & Hausler 2006).…”
