A New Treatment for Severe Pulmonary Embolism

Abstract: This preliminary results suggest that this easy technical method may be useful in the treatment of life-threatening pulmonary embolism in patients with absolute contraindications to thrombolytic therapy. A larger cohort of patients is necessary to determine whether this treatment should be proposed as an alternative to the use of fibrinolytics in selected patients.

“…16 Several mechanical or rheolytic thrombectomy devices not designed for use in large main pulmonary arteries have been investigated in small PE cohort studies (Table 3). [17][18][19][20][21][22][23] The Aspirex device, a highly effective mechanical thrombectomy catheter, has been specifically developed for the treatment of massive PE. 24 The central part of the Aspirex device is a protected, high-speed rotational coil that aspirates, macerates, and removes thrombus through an L-shaped aspiration port at the catheter tip ( Figure 2).…”

“…L'Amplatz device determina una macerazione del trombo con rotatore ad alta velocità, con lo svantaggio di provocare una microembolizzazione distale. Molti dei dispositivi attuali sembrano essere efficaci, sicuri e potenzialmente salvavita in presenza di un grosso trombo fresco [18][19][20][21] . Comunque, nessuno dei dispositivi è stato studiato in un ampio trial clinico controllato 1 e tutti i dispositivi disponibili in commercio presentano ancora delle limitazioni.…”

“…This pulse-spray treatment facilitates standard thrombectomy performed after a pause of 15 to 30 minutes to allow the thrombolytic agent to take effect. Since the initial description of this device used or treatment of PE by Koning et al in 1997, 19 the published experience comprises Ͼ140 patients with massive and submassive PE. The largest series included 50 patients with combined catheter-directed thrombolysis used in 30% of treated patients.…”
