A new strategy in lung/lobe isolation in patients with a lung abscess or a previous lung resection using double lumen tubes combined with bronchial blockers

Abstract: The combined use of a double-lumen tube and a bronchial blocker can be very helpful in two different clinical scenarios: (1) in isolating not only the contralateral lung, but also the lobe/s of the same lung in which the infected lobe must be resected, (2) in preventing/treating hypoxemia because of the presence of a contralateral lobectomy. A cardiothoracic anesthesiologist must expertise this technique to avoid complications during surgery.

“…However, in thoracoscopic lobectomy, these two options only protect the non-operative lung from contamination and do not protect the non-operative lobe(s) of the operative lung. Recent case reports have explored the protection of non-operative lobe(s) of the operative lung using solutions such as a DLT + BB or the double-bronchial blocker (DBB) technique [3][4][5] . These plans are feasible and can provide lung protection for nonsurgical lung lobes.…”

[3][4][5] These plans are feasible and can provide lung protection for nonsurgical lung lobes. However, given the small sample sizes of these previous reports, the feasibility and e cacy of using DLT + BB or DBB techniques in thoracoscopic lobectomy still need to be determined.…”

“…I read, with great interest, the article by Gil et al . [ 1 ]—‘a new strategy in lung/lobe isolation in patients with a lung abscess or a previous lung resection using double lumen tubes combined with bronchial blockers.’ I congratulate the authors for the successful management of seven cases where the combination of double lumen tube (DLT) and bronchial blocker (BB) was used for ‘selective lobar blockade.’…”
