A new risk grouping for IgD myeloma based on analysis of 165 Japanese patients

Abstract: The common staging systems for myeloma do not include IgD myeloma because its rarity makes analysis difficult. We studied a series of 165 Japanese patients with IgD myeloma retrospectively to assess which of the easily assessable parameters present at the time of diagnosis were of prognostic significance. The important individual variables detected in a previous univariate analysis were placed in a multiple regression model to identify the major prognostic factors for survival. This analysis showed that light‐… Show more

“…IgD myeloma accounts for approximately 2% of all myeloma subtypes [1,3,4]. It can easily be misdiagnosed as light-chain only disease, because there may be no definitive M-protein band as in other subtypes [5][6][7][8].…”

“…They observed that an M-spike was visible in only 60%, k light-chain disease in 60%, Bence-Jones proteinuria in 96%, renal failure in 33%, and hypercalcemia in 22%. Shimamoto et al [4] reviewed 165 patients with IgD myeloma and on multivariate analysis identified two prognostic factors, k light-chain and white blood cell count >7 3 10 9 , which helped stratify patients into three prognostic groups (0, 1, or 2 risk factors) affecting PFS. These large studies highlight the role of k-light chain as an adverse prognostic marker.…”

“…1 Survival of patients with IgD myeloma is generally accepted to be poorer than that of patients with 'common' myelomas. [2][3][4][5] The situation for non-secretory myeloma is less clear; originally thought to confer an adverse prognosis the majority of authorities now suggest that the prognosis of patients with this form of myeloma is similar [6][7][8] to or possibly better 9,10 than that of patients with common myelomas. It should be noted that these results are based on only small numbers of cases in the series reported.…”

“…By contrast, these numbers in IgG multiple myeloma are 59 and 15%, respectively. Patients are usually younger than those with IgG or IgA myeloma and are more likely to present with nonspecific systemic symptoms, a more rapid progressive clinical course, a higher rate of extramedullary involvement and shorter survival time (30,31). Unfortunately, the majority of clinical laboratories do not include IgD in the initial workup of monoclonal gammopathies (32).…”
