A new radiologic measurement for the diagnosis of Haglund's deformity

Abstract: The authors describe a new method of radiologic measurement of "Haglund's deformity", based on a radiologic study of 31 feet operated for posterior heel pain and more especially for calcaneal tendinopathies related to deformity of the calcaneus, and on a series of 60 asymptomatic feet. This angular approach requires a lateral weight-bearing view and proved positive in 85% of symptomatic feet, with only 14% of false-positives in the control group. It thus proves markedly superior to previously described radiolo… Show more

“…However, bony over resection places patients at great risks of lesion of the Achilles tendon insertion and calcaneus fracture [4]. Various radiographic measurements are used to judge the extent of the required bony correction [12,[19][20][21]. But there is absence of a perfect radiographic measurement [6,8,14,22].…”

“…The Fowler and Philip angle (FPA) (5) and the calcaneal pitch angle (CPA) (6) have frequently been used. However, the relation between symptomatic Haglund's deformity and the measuring methods, especially the FPA and CPA, is poor (7,10,11). Some criteria, established by Chauveaux et al (7), are fundamental to the measuring methods (i.e., the degree of anatomic deformity, its site in the sagittal plane, and, especially, the position of the calcaneus in relation to the plane of the ground).…”

Some criteria, established by Chauveaux et al (7), are fundamental to the measuring methods (i.e., the degree of anatomic deformity, its site in the sagittal plane, and, especially, the position of the calcaneus in relation to the plane of the ground). Other variations have been proposed to be associated with Haglund's deformity, such as the step spur and hypertrophy of the plantar osseous projection (11,12).…”

“…Upper and lower parallel pitch lines are drawn parallel to the longitudinal axis of the calcaneal body using a lateral weight-bearing view of the foot. 38 Patients with prominent reactive exostoses extending above the upper line may benefit from tendon debridement and exostectomy.…”
