Background: Adhesions due to pelvic/abdominal surgery are a common serious pathology possibly entailing severe complications. This study investigates the adhesion prevention capability of the novel starch-based agent 4DryField® PH, which together with saline solution forms a barrier gel. Herein, an optimized adhesion model (OPAM) inducing severe adhesions/agglutinations with high reproducibility was used. Methods: In 19 Lewis rats, a 1 × 2 cm abdominal wall defect was created, the peritoneum of the neighboring cecum was abraded, and both injured areas were approximated by suture. Rats were randomized to control (n = 10) or 4DryField PH treatment (n = 9) groups. Another 8 rats had sham surgery for safety assessment of 4DryField PH. At day 7, the quantity and quality of adhesions were assessed macro-/microscopically and evaluated statistically. Results: 4DryField PH treatment significantly reduced the incidence and severity of adhesions as verified by significantly improved adhesion scorings (0.4 vs. 4.5; 1.1 vs. 9). Histology revealed reconstitution of the cecum and abdominal wall including regeneration of the visceral/parietal peritoneum. In sham-operated rats, 4DryField PH did not induce adhesion formation. Conclusions: 4DryField PH gel was highly effective in preventing adhesions. Histologically, the injured cecum and abdominal wall regenerated well in the presence of 4DryField PH. Considering the severity of OPAM trauma, the potential of 4DryField PH to prevent adhesions can be rated excellent.

Peritendinous adhesions are serious complications after surgical repair of tendons and can lead to poor functional outcome. In the present study, the tensile strength of repaired tendons was measured biomechanically in two groups using a rabbit Achilles tendon model, and the effects of Adcon gel on peritendinous adhesions and tendon healing were examined by means of histological and mechanical analyses. In the treatment group, the Achilles tendon was transected, followed by a primary repair using a modified Kessler technique, and Adcon gel was injected between the tendon and skin of the right leg. The same operation was done for the control group, and 1 mL of normal saline solution was applied locally in a similar fashion. The experimental protocols were approved by the local animal ethics committee. Microscopic evidence of the formation of adhesions and of inflammation was less in the treatment group than in the control group. There was no significant difference in the tensile load necessary to rupture the repaired tendons between the two groups. Adcon gel reduced the peritendinous adhesions histologically without impairing tendon healing, as determined with mechanical analyses. The use of Adcon gel may provide a simple means of preventing of postoperative peritendinous adhesions, thereby offering a beneficial effect on tendon repair.

We evaluated the efficacy of a biomaterial composed of oxidised regenerated cellulose (Divide) in decreasing the formation of peri-tendinous adhesions in an experimental and clinical study. The experimental study was carried out in 47 New Zealand white rabbits whose Achilles tendons were subjected to a crush injury. The left Achilles tendon served as a control, while the right tendon was wrapped in a sheet of Divide Ò . Six weeks later the range of motion at the ankle was measured, the tendons were histologically analysed for peri-tendinous adhesions, the tendon sheath was examined using a scanning electron microscope for determining resorption of the biomaterial and the mechanical properties evaluated. The clinical study was performed on 22 patients; 16 requiring post-traumatic extensor tenolysis and six who underwent dorsal synovectomy for rheumatoid arthritis. A sheet of Divide Ò was placed either between the periosteum and the tendon or around the tendon in all patients. Total active motion and grip strength was evaluated in all patients pre-operatively and at regular intervals post-operatively. The experimental study showed that ankle range of motion was similar between the control and Divide Ò treated groups in two rabbits, better in the Divide Ò group in seven rabbits and better in the control group in two rabbits. The results were not statistically significant (P B 0.262) since the numbers were quite low. Histological analysis showed that the treated tendon had a better gliding surface and minimal inflammation/reaction compared to the control group. Scanning electron microscopy on the 10th post-operative day revealed the formation of a new tendon sheath in the Divide Ò group. The mechanical properties of both the control and experimental Achilles tendon were similar and comparable to a human tendon. The clinical study demonstrated that Divide Ò was completely resorbed, perfectly biocompatible and non-allergenic. The mean T.A.M. improved from 159.4°to 187.3°(P B 0.074) and grip strength from -26.7 kgF to 10.7 kgF on the 84th post-op day in the extensor tenolysis group (16 digits), while mean return of function to pre-operation level was achieved at 42nd post-operative day and grip strength increased from -3.4 kgF in the immediate, post-operative period to +5.7 kgF at the 84th day in the rheumatoid arthritis group.
