1981-2010, et 65 % depuis la commercialisation du premier anticancéreux dans les années 1940 [1]. La nature et sa diversité chimique gigantesque constituent donc un réservoir immense de molécules bioactives au potentiel thérapeutique vaste. Un « coffre aux trésors » de molécules à identifier, produire et appliquer aux pathologies qui nous touchent ! Le chemin qui conduit d'une substance naturelle produite par un organisme vivant (plante, invertébré, bactérie) jusqu'à l'arrivée sur le marché d'un médi-cament efficace n'est pas un long fleuve tranquille ! Ce long parcours, où le succès fait figure d'exception, s'étend sur de nombreuses années (plus de 12 années en moyenne) et requiert des dépenses considérables (près d'un milliard d'euros). La purification et la détermination de la structure chimique d'une substance naturelle, processus en soi déjà très complexes, n'en sont que la toute première étape. La molécule purifiée (ou resynthétisée car le problème d'approvisionnement est souvent un premier obstacle) subit de nombreux tests précliniques (souvent éliminatoires) : criblages moléculaires et/ou cellulaires, identification du mécanisme d'action moléculaire, optimisation chimique (relation structure/activité), études sur protéines purifiées (cristallisation, interaction, effet sur une activité enzymatique, etc.), modèles cellulaires et modèles animaux de la pathologie, études de toxicité, sélection d'une voie d'administration optimale, études pharmacocinétiques, métabolisme, Depuis les années 1940 jusqu'à aujourd'hui, les composés naturels ou issus de composés naturels repré-sentent 55 % des produits mis sur le marché en oncologie. Selon Newman et Cragg [1], si l'on considère uniquement les petites molécules (sont exclus les vaccins et les composés biologiques), ce pourcentage atteint près de 80 % des médicaments pour la période Série « Pionniers de la découverte de substances naturelles à activité thérapeutique » Avant-proposLa plupart de nos médicaments sont issus, dérivés ou inspirés de substances naturelles. Leur identification initiale, leur synthèse chimique, leur production industrielle, leur développement pharmaceutique et leurs applications thérapeutiques variées sont, pour chacun, une histoire exemplaire unique, le fruit de longs travaux parsemés d'échecs, de découvertes inattendues. Ils sont le fruit d'un mélange de chance, de sagacité et de ténacité qui permet parfois aux recherches les plus fondamentales de trouver des applications majeures. Dans cette série d'articles d'histoire des sciences qui commence par Pierre Potier, nous illustrerons le parcours scientifique de chercheurs de renom qui ont mis leur énergie, leur enthousiasme, leur imagination, leur génie et leur patience au service de la découverte de substances naturelles à activité thérapeutique.

Die Entwicklung einer oralen Darreichungsform von Vinorelbin (Navelbine ® Weichkapseln, Pierre Fabre Pharma, Freiburg i.Br., Deutschland) ist als großer Fortschritt in der Behandlung von Krebspatienten zu werten. Eine orale Chemotherapie ist für Patienten angenehmer und mit erheblichen Zeiteinsparungen verbunden. Vinorelbin wird nach Einnahme der Weichkapseln rasch resorbiert. Die Bioverfügbarkeit wurde mit 33 bzw. 43% ermittelt; sie wird durch gleichzeitige Nahrungsaufnahme oder Erbrechen (=1,5 h nach Einnahme) nicht beeinflusst. Die Pharmakokinetik von oralem Vinorelbin unterscheidet sich bei jüngeren und älteren Patienten (=70 Jahre) nicht. Empfohlen wird, die Behandlung mit drei wöchentlichen Dosen von oralem Vinorelbin zu 60 mg/m2 zu beginnen (Zyklus 1) und anschließend wöchentlich 80 mg/m2 zu verabreichen. Kommt es allerdings während des ersten Zyklus zu einer schweren Neutropenie, wird die Behandlung mit 60 mg/m2 pro Woche fortgesetzt. Aus Untersuchungen der Bioverfügbarkeit geht hervor, dass orale Dosen von 60 bzw. 80 mg/m2 intravenösen Dosen von 25 bzw. 30 mg/m2 entsprechen. Zahlreiche klinische Studien haben inzwischen bestätigt, dass die neue orale Darreichungsform von Vinorelbin sicher angewendet werden kann - auch bei betagten Patienten - und in ihrer Wirksamkeit beim fortgeschrittenen nichtkleinzelligen Bronchialkarzinom (NSCLC) und metastasierten Mammakarzinom (MBC) der von intravenösem Vinorelbin nicht nachsteht. In einem randomisierten Phase-II-Vergleich von oralem Vinorelbin in dem empfohlenen Dosierungsschema und intravenösem Vinorelbin (30 mg/m2) wurden bei Patienten mit fortgeschrittenem NSCLC keine signifikanten Unterschiede bei Ansprechrate sowie progressionsfreiem und Gesamtüberleben festgestellt. In Studien mit einem Kombinationsregime von Vinorelbin und Cisplatin oder Carboplatin beim fortgeschrittenen NSCLC bzw. von Vinorelbin und Taxanen, Capecitabin, Epirubicin und dem HER2/neu-Antikörper Trastuzumab beim MBC ließen sich patientenfreundlich und ohne Wirkungseinbuße intravenöse Vinorelbin-Dosen ganz oder teilweise durch orale Vinorelbin- Gaben ersetzen. Die simultane Chemoradiotherapie mit oralem Vinorelbin und Cisplatin erwies sich als gut verträglich und ermöglichte bei Patienten mit lokal fortgeschrittenem NSCLC ein relevantes Down- Staging. Eine neue Chemotherapieform mit optimierter antiangiogenetischer Wirkung ist die «metronomische» Therapie. Die ersten Ergebnisse einer entsprechenden Studie mit oralem Vinorelbin jeden zweiten Tag in niedrigen Dosen bei Patienten mit therapierefraktären soliden Tumoren liegen bereits vor. Orales Vinorelbin ist auch als Alternative für Patienten geeignet, bei denen es unter Behandlung mit intravenösem Vinorelbin zu einem akuten Tumorschmerzsyndrom kommt.

Phase II Study of Vinorelbine in Heavily Previously Treated Small Cell Lung Cancer

Twenty-four previously treated patients with refractory or relapsed small cell lung cancer (SCLC) were entered into a prospective, multicenter phase II study. All 24 patients had been pretreated with some form of cisplatin-based chemotherapy. The median time of chemotherapy was 4.2 months (range 1.4–9.4 months). Patients were treated with a dose of 25 mg/m² of vinorelbine weekly. Twenty-four patients were eligible for response and for toxicity. Partial response was observed in 3 out of 24 eligible patients (12.5%; 95% confidence interval, 2.7–32.4%). All 3 patients who responded had previous chemotherapy including vincristine. The most common toxicity was leukopenia (91.7%, 66.7% in WHO 3-4 grade) and anemia (70.8%, 20.8% in WHO 3 grade). Nonhematological toxicities were moderate and mild. These results support a two-stage sequential study design of previously untreated patients for further phase II study in SCLC.
