A New Approach to the Management of Fixed Flexion Deformity of the Knee Using Ilizarov’s Principle of Distraction Histogenesis: A Preliminary Communication

Abstract: Fixed flexion deformity (FFD) of the knee causes significant patient disability and is challenging to orthopedic surgeons because of its complexity. A prospective study was conducted using Ilizarov's principles of distraction neohistogenesis and differential distraction and Ilizarov's ring fixator for the correction of this deformity in cases with soft-tissue contracture. This was done without any concurrent bony procedures. A prospective, uncontrolled study of 26 patients (39 knees) was conducted over a 4-yea… Show more

“…Neurovascular injury is also a relatively common serious complication of external fixator distraction 22,23 . The Ilizarov technique distracted the joint slowly and steadily at a speed close to that of nerve regeneration, which could reduce the risk of neurovascular complications 24 . In this study, the distraction speed was adjusted according to the patient's compliance or whether there were neurological complications.…”

The pulse of the dorsalis pedis artery, the sensory changes of the lower limbs, the tension of the popliteal skin and the needle eye should be given close attention before and after each distraction.…”

“…Makhdoom et al 36 observed pin-tract infection in 6 of 27 clubfoot deformities (22%). Gaurav and Vilas 37 found that pin-tract infection occurred in 2 (5%) of 26 patients with fixed flexion deformity of the knee. In the present study, superficial pin-tract infections were recorded in 2 of 6 patients.…”
