A Nationwide Survey of Mortality in Acromegaly

Abstract: The posttreatment basal GH concentration less than 2.5 microg/liter in acromegalic patients is associated with a normal lifespan. Excess mortality is confined to poorly controlled patients and possibly those who have received conventional radiotherapy.

“…Nevertheless, in the subgroup of irradiated patients, mortality was significantly increased (SMR 2.7), confirming data reported by Ayuk (SMR 1.58) and from Finland (SMR 1.69) (5). In contrast, the mortality observed in the non-irradiated patients in all three studies was not different from the general population (SMR of 0.92, 0.95 and 0.94 respectively).…”

“…Median age at death in the Belgian survey (68 years in men and 74 years in women) is higher than reported in previous series: 60 years in REA (12), 61 years in Quebec (8) and New Zealand (6), but close to the 64 years for men and 69 years for women in Finland (5) and the 66 years in the West Midlands database (4). By comparison, current life expectancy in Belgium is 75.6 in men and 81.7 years in women.…”

“…In patients with more advanced disease, successful therapy eliminates or resolves all manifestations of the disease in a minority, and diminished qualityof-life persists in the majority (24). Overall morbidity and mortality, related primarily to chronic cardiovascular disease, is a function of biochemical control: risk of mortality may be as high as 3.5-fold greater in patients with persistent disease compared with those in remission (22,23,(25)(26)(27)(28). Although molecular diagnosis of PAP in unselected pituitary adenomas would be first performed in a research setting, thousands of paraffin-embedded somatotropinoma samples in the United States alone are available for prescreening of PAP by AIP IHC, pending consent from the patients.…”
