A National Survey Investigating UK Prescribers' Opinions on Chemotherapy Dosing and ‘Dose-Banding’

Abstract: There was general support for dose-banding, but concerns about the lack of an evidence base could be a barrier to the wider introduction of the system. Consequently, more clinical studies are required to justify the safety and efficacy of dose-banding, and also to evaluate whether dose-banding is acceptable within clinical trials.

“…Since the definition of this process in 2001 by Plumridge and Sewell [13], it has globally enabled to improve the organisation of the anticancer drugs preparation unit, through a simplification of the prescription analysis and a reduction of preparation errors [21][22][23][24][25][26][27]. Our system presents a major difference with the UK concept, where ''a range of pre-filled syringes or infusions, manufactured by pharmacy staff or purchased from commercial sources, can then be used to administer the standard dose''.…”

“…Literature published on opinions of pharmacists, oncology nurses, oncologists and haematologists on dose-banding were also taken into consideration. 12,32 The questionnaire was peer-reviewed by two senior pharmacists who were not directly involved in the project. Pharmacists and registrars (senior grade of non-consultant hospital doctor) working in haematology–oncology and nurses working on the HODW in SJH were invited to participate in the survey.…”

“…For each band a standard dose (mid-point of band) is provided with standard, premade syringes or infusion bags used singly or in combination to obtain a standard dose of maximum ±5% of the corresponding calculated BSA individualized dose. This concept has been proven benefits on improving workload for pharmaceutical team and nurses without measurable decrease in treatments outcomes of cancer patients [6,7]. Some widely used cytotoxic drugs have been previously dose-banded or dose-standardized [8][9][10] (i. e., carboplatin, cisplatin, paclitaxel, docetaxel, gemcitabine and monoclonal antibodies [10]).…”
