A multilevel approach for promoting physical activity in rural communities: a cluster randomized controlled trial

Abstract: BackgroundPhysical activity (PA) has demonstrated a decreased risk in various cancers and other chronic diseases; however, rural residents are less likely to attain recommended levels of PA compared to urban and suburban counterparts. Given rural residents make up 15% of the United States population, there is a need for novel approaches to increase PA among this population. The goal of the present study is to investigate the effectiveness of a multilevel intervention to increase PA rates among rural residents.… Show more

“…The communities were chosen as part of a larger intervention study, based on location, population size, and presence of at least one local walking trail. Walking trail used to promote PA is part of this larger study intervention and therefore a focus of this current study [26]. Rurality was defined by the US classification system whereby each community was at the nonmicropolitan level (less than 9,999) or micropolitan level (10,000 to 49,999) [27].…”

“…These types of strategies are recommended and have proved effective to successfully increase physical activity behaviours [47,48]. Although there appear to be setting-specific barriers to physical activity in rural areas, such as lack of sports infrastructure and public parks [46], there is a paucity of interventions addressing these barriers [49]. Additionally, it is unclear if the interventions aimed at increasing physical activity in urban settings are scalable to rural locations [47].…”

“…Overall, a structured pickleball program appears to be a promising way to motivate this target population to engage in physical activity. This study's findings are especially relevant given the paucity of evidence on physical activity interventions targeting rural communities [20,21]. One key improvement in our sample following pickleball training was increased vertical jump height.…”

“…The rural environment poses unique physical activity barriers [13] which are further aggravated by lack of transportation (to facilities) and limited social support available to rural older adults [12]. Yet, prior studies have examined physical activity promotion mainly in the context of urban settings [20,21], leaving a gap in the evidence base on the effectiveness of interventions in rural communities.…”
