A Multicenter Prospective Validation Study for the Korean Thyroid Imaging Reporting and Data System in Patients with Thyroid Nodules

Abstract: ObjectiveTo validate a new risk stratification system for thyroid nodules, the Korean Thyroid Imaging Reporting and Data System (K-TIRADS), using a prospective design.Materials and MethodsFrom June 2013 to May 2015, 902 thyroid nodules were enrolled from four institutions. The type and predictive value of ultrasonography (US) predictors were analyzed according to the combination of the solidity and echogenicity of nodules; in addition, we determined malignancy risk and diagnostic performance for each category … Show more

“…The specificity of this study was only 7.07% which was also lower compared to other studies (29%-75%). 8,[22][23][24] In our study, the prevalence of malignancy was higher at 33.56% compared to an FNAB based series wherein malignancy rate was only 3 to 7%. 7 With this, PPV/NPV might be affected since an increase in prevalence leads to increase in PPV.…”

“…[8][9][10][21][22][23][24] The histopathology report of the patient who had a malignant result showed follicular carcinoma on top of a micropapillary carcinoma. One patient who also had TIRADS 3 had an incidental finding of micropapillary carcinoma measuring 0.5 cm within the left lobe but the FNAB was done on another nodule that The accuracy of TIRADS in our study was 53% which was slightly lower as compared to the studies of Ha et al, at 69.5% 22 and Russ et al, at 62%. 25 The reason for low accuracy is possibly because of a high false positive rate.…”

“…These tumors have been diagnosed as high- or intermediatesuspicion nodules using US based on the Korean Thyroid Image Reporting and Data System [23,24]. Some tumors have been reported to have a well-defined border and could be predicted to exhibit indolent behavior [25].…”

“…Conventional US can be used to detect nodules, and it also provides information on the composition, size, and number of nodules, as well as their anatomical relationships with surrounding structures. US findings that have been reported to be suggestive of malignant thyroid nodules include solid composition, hypoechogenicity or marked hypoechogenicity, a microlobulated or irregular margin, mixed calcification or microcalcifications, and a taller-than-wide shape [1,2]. However, a considerable number of US findings overlap between benign and malignant thyroid nodules, making it difficult to identify thyroid malignancies based on US imaging alone [3,4].…”
