A Modified SpeedBridge Technique for Retracted or Delaminated Rotator Cuff Repairs

Abstract: Treatment of full-thickness rotator cuff tears vary in surgical technique dependent on the amount of retraction of the rotator cuff and/or delamination of the soft tissue. The described technique addresses both of those concerns. We present a modification of the SpeedBridge technique used to address retracted or delaminated repairs and effectively expand the indications for use of the double-row knotless technique. In this modification, the reduction is performed by an initial anchor with several stay sutures … Show more

“…11 Previous authors have also described the use of 3 medial anchors for large and massive tears. 29,30 However, as mentioned previously, these approaches may be limited by available footprint per anchor size. Goschka et al 31 compared the traditional hard body anchors with all-suture anchors determined that the latter reduces the occurrence of loose body complications as it requires a decreased amount of bone removal.…”

“…These sutures are cinched over the medial tendon stump as has been previously described. 6 Next, the free ends of the locked reduction sutures are shuttled through the lateral portion of the cuff remaining on the greater tuberosity utilizing a SutureLasso (Arthrex Inc.) ( Fig 5 ). Attention is taken to optimize reduction of the intratendinous tear just lateral to the musculotendinous junction.…”

“…The locking looped reduction sutures are attached laterally to a central reduction to reduce the rotator cuff and graft to the greater tuberosity as has been previously described. 6 The remaining FiberTapes are then placed in a crisscrossed fashion to lateral knotless anchors, SwiveLock (Arthrex Inc.), completing a double-row SpeedBridge construct to add compression to the repair ( Fig 8 ).
