A model for training and evaluation of myringotomy and tube placement skills

Abstract: Junior surgical residents performed an M&T procedure using a novel, low-cost model. This study demonstrated the skills lab's positive effect on training as measured by global rating scale, time-to-completion, and overall resident confidence. We anticipate its application to be valuable not only in training residents but also in assessing competency.

“…Simulation is emerging as a mandatory component of surgical training and a means of demonstrating surgical competency. Malekzadeh et al (8), using a simplistic myringotomy and ventilation tube insertion skills box, demonstrated the laboratory skills' positive effect on training, as measured by a global rating scale, time to completion, and overall resident confidence. Wiet et al (9) demonstrated no statistical difference between practices on a virtual temporal bone dissection system compared with practice on human cadaveric temporal bones.…”

“…Therefore, residents spent an inordinate amount of time working to develop these skills. The combination of surgical experience and laboratory cadaveric dissection has resulted in otologists of excellent quality (8).…”

“…Numerous models have been described ranging from DIY models based on readily available materials 12 to more complex physical models 13,14 . In addition, a VR simulation model for myringotomy and tube insertion with supporting haptic feedback has been developed 15–18 .…”

“…In addition, a VR simulation model for myringotomy and tube insertion with supporting haptic feedback has been developed 15–18 . Although global rating scales and task-based checklists have been developed and validated 12,19 to assess for myringotomy with tube insertion competency, evidence to support the efficacy of myringotomy simulation training is limited and high quality studies examining the transfer of skills to improved patient outcomes are needed (see SimTube project below).…”
