A model for size and age changes in the pharmacokinetics of paracetamol in neonates, infants and children

Abstract: Aims The aims of this study were to describe paracetamol pharmacokinetics in neonates and infants. Methods Infants in their ®rst 3 months of life (n=30) were randomised to sequentially receive one of three paracetamol formulations (dose 30±40 mg kg x1 ) over a 2 day period. The formulations were (a) elixir, (b) glycogelatin capsule suppository and (c) triglyceride base suppository. Approximately six blood samples were taken after each dose over the subsequent 10±16 h. Data were analysed using a nonlinear mixed… Show more

“…This model is empirical and does not extrapolate to adult values because, physiologically, clearance must rise and plateau at some age. A first-order model, common in biological systems, has been previously used to investigate clearance maturation of other drugs during infancy [14]. A 'mature' clearance can be estimated (Figure 1c) at plateau:…”

“…Allometric scaling has a strong theoretical and empirical basis with a PWR exponent of 0.75 for clearance and 1 for distribution volume [10][11][12][13][14]. We have chosen a standard weight of 70 kg to enable comparison with adult values and other paediatric studies.…”

“…Of note, weight has been identified as a primary covariate of the CL and V of several drugs in neonates, including amikacin (12), tobramycin (17), vancomycin (14,16), paracetamole (5), and cisapride (37). Few studies have reported further improvement of the models when gestational age, postconceptional age, and postnatal age were included in addition to WT (5,13,35). Levels of binding of AMB and ABLC to albumin, lipoproteins, and other proteins may differ between neonates and adults.…”

“…13 Additionally, activated carbon was used in this patient to decrease the absorption of the toxic agent, which is slow according to pharmacokinetic data in children. 14 We did not prescribe it to our patient because the first dose had been administered over 12 hours before the error was discovered.…”
