Home healthcare service provides daily medical services at patients' homes. The service aims to satisfy the patients' requirements which must be done by one or more qualified staff visiting them in right time. This paper proposes a new framework integrating the heuristic search and fuzzy logic with knowledge acquisition to develop reliable and effective decision to match qualified staff to offer homecare services with the minimum cost and better preference consideration under feasible time-slots. The framework is designed based on a hierarchical approach which divides the staff selection system into two layers: high and low level. The high level is responsible for retrieving information, such as patients' locations, service Title 2 distance and operation cost as well as dealing with constraints such as patients' requirement and working time-slots. This information is passed to the lower level which is employed with fuzzy logic to estimate a set of the level of staff selection before returning results back to the higher layer to create the service plan. Our empirical study shows that the proposed framework has a potential to provide improved solutions with minimized operating cost and improved patient's satisfaction. The case study for the experiment comprises of 40 medical tasks with 8 service routes and 8 carers under time-slot windows.

In recent years, the management of health systems is a main concern of governments and decision-makers. Home health care is one of the newest methods of providing services to patients in developed societies that can respond to the individual lifestyle of the modern age and the increase of life expectancy. The home health care routing and scheduling problem is a generalized version of the vehicle routing problem, which is extended to a complex problem by adding special features and constraints of health care problems. In this problem, there are multiple stakeholders, such as nurses, for which an increase in their satisfaction level is very important. In this study, a mathematical model is developed to expand traditional home health care routing and scheduling models to downgrading cost aspects by adding the objective of minimizing the difference between the actual and potential skills of the nurses. Downgrading can lead to nurse dissatisfaction. In addition, skillful nurses have higher salaries, and high-level services increase equipment costs and need more expensive training and nursing certificates. Therefore, downgrading can enforce huge hidden costs to the managers of a company. To solve the bi-objective model, an ε-constraint-based approach is suggested, and the model applicability and its ability to solve the problem in various sizes are discussed. A sensitivity analysis on the Epsilon parameter is conducted to analyze the effect of this parameter on the problem. Finally, some managerial insights are presented to help the managers in this field, and some directions for future studies are mentioned as well.

Providing appropriate home health care is one of the increasing concerns in the health care organizations. Home Health Care provides various services for disabled or elderly individuals at their homes. Also, deal with the current critical situation of the coronavirus disease (COVID-19) due to the limited capacity of hospitals and the feeling of insecurity in crowded places, home health care is more recommended. This paper addresses a Home Health Care Routing and Scheduling Problem (HHCRSP) with two modes of transportations including public and private modes. Also, multi-depot version of the problem is studied to enhance the service delivery in scattered points. In this study, a mathematical model is presented based on a Mixed Integer Linear Programming (MILP) whose objective function is minimization of the sum of the travel distance and overtime costs. Furthermore, three meta-heuristic algorithms including Invasive Weed Optimization (IWO), Grasshopper Optimization Algorithm (GOA) and Simulated Annealing (SA) are presented for solving the large-sized problems. Since the performance of meta-heuristic algorithms depends on setting the parameters, Taguchi method is used to statistically set parameters of the developed algorithms. The computational results have shown that the proposed IWO has worked better than the other two proposed algorithms statistically.
