Causal interactions and correlations between clinically-relevant biomarkers are important to understand, both for informing potential medical interventions as well as predicting the likely health trajectory of any individual as they age. These interactions and correlations can be hard to establish in humans, due to the difficulties of routine sampling and controlling for individual differences (e.g., diet, socio-economic status, medication). Because bottlenose dolphins are long-lived mammals that exhibit several age-related phenomena similar to humans, we analyzed data from a well controlled 25-year longitudinal cohort of 144 dolphins. The data from this study has been reported on earlier, and consists of 44 clinically relevant biomarkers. This time-series data exhibits three starkly different influences: (A) directed interactions between biomarkers, (B) sources of biological variation that can either correlate or decorrelate different biomarkers, and (C) random observation-noise which combines measurement error and very rapid fluctuations in the dolphin’s biomarkers. Importantly, the sources of biological variation (type-B) are large in magnitude, often comparable to the observation errors (type-C) and larger than the effect of the directed interactions (type-A). Attempting to recover the type-A interactions without accounting for the type-B and type-C variation can result in an abundance of false-positives and false-negatives. Using a generalized regression which fits the longitudinal data with a linear model accounting for all three influences, we demonstrate that the dolphins exhibit many significant directed interactions (type-A), as well as strong correlated variation (type-B), between several pairs of biomarkers. Moreover, many of these interactions are associated with advanced age, suggesting that these interactions can be monitored and/or targeted to predict and potentially affect aging.

Chronic kidney disease (CKD), also known as renal failure, primarily affects people with a history of hypertension and diabetes. In this medical condition, the rate of filtration or kidney function can decrease over the course of a month or a year. Several studies have found an increase in pro-inflammatory cytokines in patients with CKD. Platelet dysfunction and hyperreactivity have been linked to chronic inflammation. When the platelet count exceeds 450,000/l, thrombocytosis occurs. It is also referred to as thrombocythemia. Reactive thrombocytosis accounts for 80-90% of all thrombocytosis events, with inflammation being one of the most common causes. We report a 67-year-old man presenting to the hospital with fever and weakness. The patient had a history of CKD stage V and currently was rceiving hemodialysis treatment. Laboratory finding showed patient had thrombocytosis. Further examination is required to determine the cause of thrombocytosis in CKD patient.

Developing microscopy based microfluidic SLS assay for on-chip hemoglobin estimation

Point-of-care (POC) biochemical assay is a highly important biochemical assay to estimate hemoglobin in the blood. High reagent volumes and complex-expensive optical setup requirements pose serious challenges when it comes to adopting conventional biochemical assays such as the Sodium Lauryl Sulfate (SLS) method into a POC device. Here, we report a modified SLS assay on a microfluidic platform, wherein the quantification is achieved using a simple microscopy-based imaging setup. Assay parameters, including SLS reagent-to-blood volume ratio, total reaction volume, the concentration of sodium dodecyl sulfate, and microfluidic chamber design, were optimized in order to achieve quantitation capability across a clinical range of hemoglobin using a path length suitable for the microfluidic platform. Besides quantitative correlation with a clinically accepted-validated standard method, the spectral absorption characteristics of the hemoglobin–SLS reagent mixture in the newly developed assay were compared with those of conventional SLS assays. The finalized chip design, including the reagent, cost 0.136 USD. The microfluidic chip in combination with an automated microscope was able to achieve a Pearson correlation of 0.99 in a validation study comparing the newly developed method and a commercially available hematology analyzer, with a turnaround time of 10 min, including incubation time. The clinical performance was ascertained, and the method achieved a sensitivity of 92.3% and a specificity of 53.8%. Overall, an automated microscopy-based biochemical assay was developed to estimate hemoglobin in whole-blood, using microfluidics technology, wherein the detector was a conventional camera associated with microscopy.
