A 'Made in One Piece' Skeleton in a 22-Year-Old Man Suffering from Sickle Cell Anaemia

Abstract: A 22-year-old African male with known sickle cell anaemia was referred by a Congolese medical centre with a request to improve his poor physical condition. He was unable to walk, stand or sit because his large joints and his spine were either ankylosed or very rigid. Radiographs showed joint fusion from the third to the fifth cervical vertebrae, of both hips, of the left knee, and a bilateral osteonecrosis of the humeral head. There was no scintigraphic evidence for an active osteomyelitis (99mTc-MDP (methyldi… Show more

To our knowledge, total bilateral hip and unilateral knee arthroplasty has not been described previously in the literature except in one case of an African male with a similar presentation 4 .…”

“…In these patients many young adults will be seeking treatment for avascular necrosis of the femoral head. Review of the literature showed few reports on hip replacement arthroplasties in SCD, with the number of patients ranging from one [9] to 30 [1] and a high complication rate [7,12,16,22]. The incidence of sickle cell haemoglobinopathies presents major health care problems in Bahrain [18].…”

“…The 99m Tc sulfur colloid scan has been used in a variety of clinical settings to demonstrate bone marrow elements, with the normal distribution of radiocolloid approximately 80% within the liver, 15% within the spleen, and 5% within the bone marrow. [12][13][14][15] In our patient, the radionuclide scan provided encouraging evidence that no further medical management was needed in the light of her appropriate, intramedullary hematopoiesis, and it revealed this information in a relatively painless way for the patient.…”
