A high incidence of vertebral fracture in women with breast cancer

Abstract: Metastases from breast cancer appear to have a predilection for the skeleton. In addition to focal skeletal disease, there is evidence that breast cancer is associated with generalized disturbances in skeletal metabolism.Increased resorption and accelerated turnover of bone have been shown in biopsies taken at sites distant from skeletal metastases (Taube et al, 1994), perhaps mediated by the systemic secretion of parathyroid hormone-related protein (PTHrP) which is expressed in breast cancer tissue (Orloff et… Show more

“…Women with recurrent BC showed a sixfold increase in fractures already at the beginning of the study and a 23-fold increased incidence of fractures annually, as well as a 24.5 increased relative risk. 22 Endocrine treatment In contrast to premenopausal women, treatment with tamoxifen in postmenopausal women with BC is well known to stabilize or rather increase BMD. 23,24 According to a randomised placebo controlled double-blinded trial, an increase in the BMD in women who received tamoxifen compared with an annual decrease in the control group (0.6% vs 1.0%) has been observed.…”

“…89 Patients with carcinoma of the breast who receive adjuvant chemotherapy have a higher rate of vertebral fracture than an age-matched population of patients who do not receive chemotherapy. 90 It is hypothesized that the increased bone loss, particularly in premenopausal and perimenopausal women, is one of the causes of the propensity of breast cancer to metastasize to bone.…”

“…The incidence of vertebral fractures is increased in this group, even in the absence of bone metastatic lesions (1). The breast cancer itself seems to produce various cytokines and other factors that increase bone resorption (2).…”
