The influence of cardiovascular signs (CVS) on cognitive performance was examined in 227 older adults not suffering from dementia between 75 and 96 years of age who were sampled from the community. Participants received a comprehensive physical examination that included specific evaluation of current CVS, including dyspnea, cardiac murmur, and edema in lower limbs. They were administered tests of digit span, episodic recall and recognition, verbal fluency, and visuospatial skill. CVS were found to predict performance on tests of episodic memory and visuospatial skill, after the effects of age, education, gender, relevant drug use, and mood symptoms were controlled for. Although CVS accounted for relatively little general performance variation, the findings are relevant to the understanding of normal variation in late-life cognitive performance. Finally, CVS increased with age and accounted for a sizable proportion of the age-related cognitive variation.

We investigated cognitive impairment in a study of the health of the elderly population of Saskatchewan. Surveys of elderly persons living at home (n = 1267) and living in long-term care facilities (n = 990) were conducted in 1981. Cognitive impairment was assessed by a short 10-item mental status questionnaire previously validated against a clinical diagnosis of dementia in an elderly Canadian population. The prevalence of clinically significant cognitive impairment was found to increase with age and with dependence level in long-term care facilities. We estimate that 7.8% of the elderly population have cognitive impairment consistent with a clinical diagnosis of dementia. Our estimates are compared with those derived from other studies. These findings affirm the importance of dementia as a cause of dependence in the elderly and the need for long-term care facilities to deal with dementia and its consequences. Also, since at least as many persons with cognitive impairment live at home as in long-term care facilities, health care planners must direct attention to the elderly with dementia at home.

Retirement Status Predicting Health Conditions 16 Years Later

All 67-year-old pensioners in a primary care district (N = 142) participating in a multi-disciplinary population study were followed until the age of 83. At 83 years of age, 65 persons had survived and continued to take part in the study. Social, psychological and medical factors predicting survival during the period have previously been reported (Samuelsson et al. 1992). In the present analysis, the same variables at age 67 were used to predict health, measured with six different health indicators, at 83 years. The analysis has been performed separately for women and men. Variables at 67 years of age as determinants for health at age 83 have been identified and ranked through successive selection in a step-wise discriminatory analysis. For women, reported diseases at 67 was a very strong predictor but quite the contrary for men. Blood pressure and sleep medication were strong predictors for men but not for women. Psychological factors were more frequently included in the predictive models for women than for men. Social factors were of comparatively less importance for both men and women. There was greater accuracy in the prediction of health for women. The individual variables most often included in the predictive model for women were coping and reported health at age 67. For men, blood pressure, sleep medication and intelligence were the most frequent predictors. The analysis demonstrated clear sex-specific prediction patterns. When comparing predictors for survival and predictors for differentiated health in the same population no similarities were found.
