Introduction The completion of an integrated PET/MR prototype system for brain imaging is the latest step in the evolution of positron emission tomography. Early images with this new imaging system demonstrate that highresolution multiparametric studies can be combined without significant loss of performance of either imaging modality. Objective This new technology will make fusion of morphological and biological information much easier, yield real-time assessment of complementary variables and will provide dynamic information for kinetic model- IntroductionSince the first prototypes [1, 2] and the first commercial system [3], positron emission tomography (PET) has developed to multiring systems permitting high resolution and three-dimensional imaging of various physiological, functional and molecular targets. The first applications of PET were in brain research, and despite the many other diagnostic indications, particularly in oncology and cardiology, brain imaging remains a stronghold of PET.Therefore, this discussion of perspectives of an integrated PET/MR system is focused on research applications in neuroscience to which the combination of PET and MRI may add further impetus. This perspective is in parts speculative and it does not include a discussion of the technical or methodological challenges of combined PET/ MR imaging. These aspects are reviewed elsewhere in the articles of this supplement to EJNMMI. Development of PET for brain imagingThe progress in image quality may be followed by metabolic images of glucose consumption in one volunteer acquired on several generation PET systems over the years. The first transaxial image (Fig. 1a) was acquired on a single hexagonal array, the ECAT PET, which covered only the cortical rim. The kinetics of "C-labelled morphine and pethidine were studied by positron emission tomography (PET) at different levels of the spinal canal (C4, T4, T5, T6, L1 and L6). Studies were performed in the Rhesus monkey after intrathecal and extradural administration of the drugs at the lumbar level (L3-L4 or L+L5, seven experiments). Radioactivity 100-300 times higher than with even distribution in the body was measured initially near the site of injection for both morphine and pethidine, irrespective of the route of administration. After injection of pethidine, high activity was observed at the L6 and L1 levels, whilst the radioactive uptake was lower at T6 ( 10-20y0 of those at lumbar level). Morphine-derived "C-radioactivity showed more constant levels along the spinal canal, except at C4 where radioactivity was low. In CSE' taken from the cervical level the peaks of radioactivity of the two drugs appeared 80-1 70 min after injection. The importance of different distribution routes was quantified in a pharmacokinetic coinpartment model, using the above results. The systemic distribution was extensive, irrespective of drug or route of administration. From the site of injection the systemic distribution was at least 60 times larger than the rostra1 distribution within the spinal canal.

N-[methyl-11C] nicotine (11C-nicotine) was given intravenously to monkeys and the uptake and regional distribution of radioactivity was followed in the brain using positron emission tomography (PET). The 11C-radioactivity in the brain peaked within 1-2 min and then rapidly declined. Pretreatment with unlabelled nicotine (10 micrograms/kg) reduced the uptake of 11C-radioactivity to the brain by 30%. The uptake of radioactivity was higher following (+)11C-nicotine than (-)11C-nicotine. Both enantiomers were distributed in a similar manner within the brain. When animals were infused with a peripheral nicotinic blocker (trimetaphan) the uptake of radioactivity to the brain was lower following (+)11C-nicotine compared to (-)11C-nicotine. The amount of radioactivity was high in the occipital cortex, thalamus, intermediate in the frontal cortex and low in white matter in (-)11C injected monkeys while no regional difference in distribution of 11C-radioactivity was observed after injection of (+)11C-nicotine.
