A fatal case of JC virus meningitis presenting with hydrocephalus in a human immunodeficiency virus–seronegative patient

Abstract: JC virus (JCV) is the etiologic agent of progressive multifocal leukoencephalopathy, JCV granule cell neuronopathy and JCV encephalopathy. Whether JCV can also cause meningitis, has not yet been demonstrated. We report a case of aseptic meningitis resulting in symptomatic hydrocephalus in an HIV-seronegative patient. Brain imaging showed enlargement of ventricles but no parenchymal lesion. She had a very high JC viral load in the CSF and developed progressive cognitive dysfunction despite ventricular drainage.… Show more

“…82 Here, we show that the CPE expresses both the JCPyV attachment and entry receptors and that the virus interaction with the CPE is LSTc dependent. Recently, JCPyV VP1 was detected in the CPE in a case of JCPyV-associated meningitis, 83 demonstrating that JCPyV can productively infect these cells. Direct infection of the CPE could allow JCPyV access to brain parenchyma.…”

“…Post mortem exam showed productive JCV Infection of leptomeningeal and choroid plexus cells. Molecular analyses from the CSF JCV strain demonstrated an archetype-like RR, but no VP1 gene or agnogene mutations such as those found in cases of JCV GCN of JCVE [10]. Further studies on the role of JCV in aseptic meningitis and idiopathic hydrocephalus are warranted.…”

“…JCVE was found to be caused by a productive infection of cortical pyramidal neurons. Finally, in 2014, we observed a fatal case of JCV meningitis (JCVM) in an HIV-negative patient who had an extremely high CSF JC viral load and productive JCV infection of leptomeningeal cells [10]. In this review, we will discuss these syndromes in detail and how their discoveries have expanded our understanding of the pathogenesis of JCV in the CNS.…”

“…It is possible that neurons may be the first cells of the CNS parenchyma to be infected by JCV. The recent demonstration that JCV can cause meningeal infection (41) supports the view that the virus could spread to cortical cells first, prior to infecting glia at the GWJ and in the WM. If that is the case, chronic infection of cortical pyramidal or granule cell neurons might cause neurologic dysfunction, although absence of cytolysis may preclude detection of the lesions on MRI.…”
