Aim: To evaluate the results of the wedge resection procedure in the treatment of ingrown toenails Place and duration of study: The detailed study was carried out at Shahida Islam Medical College Lodhran and Rajib Tayyip Erdogan Hospital, Muzaffargarh, between September 2020 to August 2022. Methods: We conducted a prospective study on 73 patients who underwent wedge resection procedures between Sept 2019 to August 2021. We noted the demographic details like mean age, gender distribution, postoperative complication rate, recurrence rate, and satisfaction with the procedure. Results: The mean age(SD) was 29.3 (4.7) years with 33% males and 67% females. Wefound that 6 min was the average duration of the procedure and only two patients had postoperativecomplications; 99% were satisfied with the outcome and only 1.37% showed recurrence of the issue. Conclusion: We can conclude Wedge Resection Procedure is a safe, effective, and convenient technique in the management of ingrown toe nails. BackgroundToenails play a great part in protecting toes and peripheral soft tissues, simultaneously playing a cosmetic role. The ideal treatment should result in a functional and aesthetic outcome.ObjectiveTo describe a novel, aesthetic and minimally invasive method to treat ingrown toenail.MethodsWe retrospectively analyzed 436 lesions of 395 ingrown toes in 353 patients with a mean age of 26.0 ± 13.4 (range 10–55) from June 2014 to March 2020 in our department. A novel cosmetic approach for partial matricectomy in treating ingrown toenails was undergone. The average follow‐up time was 27.5 ± 2.8 months. The average period prior to work resumption, recurrence rate, and infection rate were measured. Mean pain Visual Analogue Scale (VAS) and Mean satisfaction VAS were used to evaluate the foot appearance.ResultsThe average period prior work resumption was 2.2 ± 2.1 days (range, 0–7 days). The recurrence rate was 1.6% (7 lesions in 6 patients) at more than 2 years of follow‐up. There was no critical complication except infection (0.46%). Mean pain VAS reduced from a preoperative score of 7.7 ± 1.5 points (range, 6–10 points) to a postoperative 3‐day score of 2.2 ± 1.0 points (range, 1–4 points; p < 0.001) while Mean satisfaction VAS improved from 1.5 ± 1.3 points (range, 0–3 points) to 9.2 ± 0.6 points (range, 8–10 points; p < 0.001).ConclusionOur proposed approach is minimally invasive relative to conventional methods, which can achieve comparable efficacy to treat ingrown toenails with granulation tissue. Therefore, it can serve as another option to treat this specific type of ingrown toenails.

Update on surgical management of onychocryptosis

Onychocryptosis is an inflammatory disease of the lateral nail fold resulting from the embedding of the lateral border of the nail plate in the periungual soft tissues with consequential formation of granulation tissue and hypertrophy of the lateral nail fold. Therapeutic options include conservative and surgical strategies. There is no consensus regarding the strategy to be adopted at each stage of severity. The bibliographic research for the elaboration of this paper was obtained through PubMed and ClinicalKey platforms. Review articles, scientific articles and descriptions of clinical cases were selected. After careful and critical analysis of the selected articles, the present literature review was carried out. The therapeutic choice for each stage of onychocryptosis is complex. Conservative treatment is safe, convenient, and inexpensive, yet it only has proven efficacy in stage I. Matricectomy with phenol or other agents after partial nail avulsion proved to be safe, inexpensive, and effective, thus being the first line in the treatment of stages II or III. Soft tissue resection techniques proved to be effective in cases of marked hypertrophy of the nail folds but are more invasive and lead to a longer recovery period. In such cases, it is recommendable to perform a resection of the periungual soft tissues, with Howard-Dubois, Vandenbos and Super U techniques being the most effective and chosen according to the location and severity of the hypertrophy of the nail folds. When treating onychocryptosis, the conservative approach is indicated for stage I, while surgical treatment is indicated for stages II and III. The first line in stages II and III should be 88% phenol solution matricectomy. New prospective comparative studies and randomized trials are necessary to implement a standardized clinical approach.
