A European perspective on testicular tissue cryopreservation for fertility preservation in prepubertal and adolescent boys

Abstract: The clinical and research questions identified and the ethical and legal issues raised are highly relevant to the multi-disciplinary teams developing treatment strategies to preserve the fertility of prepubertal and adolescent boys who have a high risk of fertility loss due to ablative interventions, trauma or genetic pre-disposition.

“…Several research groups are exploring the possibility that cryopreservation of prepubertal testicular tissue with a view to subsequent spermatogonial stem cell transplantation may preserve future fertility [41, 42]. In some animal models, this has been successful; in humans, this must still be considered experimental [43, 44].…”

“…A hererák esetén mindkét esetben felvetődik a residualis tumorsejtek viszszajuttatásának lehetősége, ezért nemzetközi ajánlások alapján nem végezhetők. Az esetleges tumoros sejtek transzplantációjának elméleti kiküszöbölésére kutatják az in vitro spermatogenesis lehetséges módszerét is [29].…”

“…Surgical recovery of pre-pubertal testicular tissue may be combined with other procedures requiring general anaesthesia such as venous catheter insertion or bone marrow sampling, minimising trauma and anaesthetic risk. 56 Whilst realisation of the fertility potential of SCCs remains as yet undemonstrated in humans, significant progress has been made in animal studies, with demonstration of restoration of functional spermatogenesis following autotransplantation of a suspension of frozenthawed SCCs into the testes of rhesus monkeys rendered azoospermic with alkylating agents 57 . Such strategies may well offer an alternative to mTESE when these and future generations of young cancer patients reach reproductive age.…”
