A Double-Blinded, Randomized Comparison of Either 0.5% Levobupivacaine or 0.5% Ropivacaine for Sciatic Nerve Block

Abstract: No studies have compared the clinical properties of levobupivacaine with those of ropivacaine when providing sciatic nerve block for hallux valgus repair. Results from this prospective, randomized, double-blinded study demonstrate that 20 mL of either 0.5% levobupivacaine or 0.5% ropivacaine provide comparable surgical block with prolonged postoperative analgesia.

“…In the literature, there is no study in which lower doses were used for unilateral spinal anesthesia. When the related studies were considered, 30 mL 0.5% levobupivacaine+10 mL 2% lidocaine were used for CSFB (11)(12)(13)(14). Because levobupivacaine does not act immediately, it was combined with a local anesthetic, lidocaine that acts, more speedily.…”

“…Bradycardia, defined as heart rate below 50 bpm, was treated with 0.5 fractional atropine doses. Sedation was evaluated throughout the observation period (between studied solutions administration and first systemic analgesic request or 24 hours post-blockade) by the Observer's Assessment of Alertness/Sedation Scale (0 = deep sedation; 20 = alertness) 12 . Anesthetic duration was defined as time in hours between blockade and first systemic analgesic request.…”

“…(7) The use of levobupivacaine is described for peripheral nerve blocks besides epidural, caudal and spinal anesthesia and the agent is used for all common indications in a wide range of clinical settings. (8,9) Regional anaesthesia is becoming increasingly popular for orthopaedic surgery as it offers several advantages over general anaesthesia and a trend towards more peripheral and selective nerve blocks exists. (10) By this aim for upper limb surgery interscalene blocks are ideally suited for shoulder and upper arm surgery, supraclavicular nerve blocks for upper arm, elbow and radial side of forearm and the infraclavicular and axillary block are suited for hand, wrist and forearm surgery.…”
