of treatment and also later, when the patient is apparently adequately treated according to the prothrombin ratio. It appears that patients with venous lesions require more anticoagulant to produce the same partial thromboplastin clottingtime than do patients with arterial lesions, and this may have a bearing on relapses and further thromboses. Examination of results from 21 patients when they were bleeding suggests that the partial thromboplastin clotting-time may be a better test for the detection of potential bleeding and bruising after overtreatment, since it is sensitive to variations in concentration of factor IX, a factor which does not affect the prothrombin clottingtime. It Disodium cromoglycate (FPL 670; Intal) is one of a series of chromone derivatives studied by Fisons Pharmaceuticals Limited. It has been shown to inhibit the effects of some specific types of reaginic antibody-antigen reactions. It is not an anti-inflammatory agent and it is not a bronchodilator, nor does it protect against the effect of histamine, of 5-hydroxytryptamine, or of slow-reacting substance (SRS-A). The compound has been shown to be relatively non-toxic.In acute toxicity tests in small laboratory animals the LD50 on parenteral administration was greater than 2,000 mg./kg. In prolonged tests no toxic effects were seen, except at doses greater than 30 mg./kg. Renal tubular degeneration was the only specific toxic effect noted at higher dosage. The compound is excreted rapidly in the bile and urine and there is no evidence in animals of accumulation in any tissue (Cox, 1967). Experimentally in atopic volunteers there was a reduction of the bronchospasm induced by aerosol of the appropriate antigen when the compound was administered by inhalation previous to challenge. The present study was designed to evaluate the efficacy of this drug in the treatment of children suffering from asthma.Method of Administration A plastic inhaler (Spinhaler) has been designed for the inhalation of the drug, which is dispensed as a micronized powder in gelatin capsules. The patient inhales the powder by taking a few deep breaths through the Spinhaler ; in practice this appliance has proved easy to use by co-operative children even with fairly severe asthma. Loughborough1 The effect on gastric acid secretion of a mast cell stabiliser FPL-52694 was examined in nine healthy volunteers. 2 The compound which is moderately well absorbed after oral ingestion, was taken for 5 days prior to the measurement of acid secretion and was also given intravenously during part of the gastric test. A submaximal dose of pentagastrin (0.3 ,g/kg/h) was given by intravenous infusion for 2 h in a controlled test and then after administration of FPL-52694. 3 There was a significant reduction (37%) in acid secretion during the second test which was probably due to the effect of the compound on mast cell degranulation in the gastric mucosa.

1 The activity of a new anti-allergic compound, I.C.I. 74,917, has been studied in the rat, mouse and guinea-pig.2 Following intravenous administration, I.C.I. 74,917 inhibits in a dose-dependent manner passive cutaneous anaphylaxis induced in rats and mice by heat-labile homocytotropic antibody. In rats, its potency is approximately 300 times that of disodium cromoglycate. 3 To achieve maximal inhibition, it is necessary to administer I.C.I. 74,917 at the same time as antigenic challenge; dosing before or after challenge has much less effect. 4 Liberation of histamine, provoked by the antigenic challenge of mast cells passively sensitized in vitro by IgE-like antibody, is reduced in the presence of I.C.I. 74,917. 5Intravenous administration of the compound has no significant effect upon local blueing reactions provoked in the rat by intradermal injection of histamine, 5-hydroxytryptamine or Compound 48/80. It has only a slight effect at high doses upon passive cutaneous anaphylaxis induced in the rat by heat-stable homocytotropic or heterologous (guinea-pig) antibodies. 6 Although not a bronchodilator in the guinea-pig, I.C.I. 74,917 partially inhibits systemic anaphylaxis. A consistent reduction in the severity of antigen-induced bronchospasm was demonstrated in the Konzett-R6ssler preparation at doses comparable to those inhibiting passive cutaneous anaphylaxis in the rat. However, there was only slight inhibition of passive cutaneous anaphylaxis in the guinea-pig. 7 I.C.I. 74,917 itself induces bronchospasm when administered to anaesthetized guinea-pigs or to a guinea-pig isolated lung preparation. This effect is reversed by salbutamol, but is not prevented by the prior administration of mepyramine, atropine or methysergide. 8 These results indicate that in the rat, mouse and guinea-pig, I.C.I. 74,917 is a potent inhibitor of certain types of immediate hypersensitivity reactions.
