BackgroundEvidence shows that the implementation of information and communication technologies (ICT) enabled services supporting integrated dementia care represents an opportunity that faces multi-pronged challenges. First, the provision of dementia support is fragmented and often inappropriate. Second, available ICT solutions in this field do not address the full spectrum of support needs arising across an individual’s whole dementia journey. Current solutions fail to harness the potential of available validated e-health services, such as telehealth and telecare, for the purposes of dementia care. Third, there is a lack of understanding of how viable business models in this field can operate. The field comprises both professional and non-professional players that interact and have roles to play in ensuring that useful technologies are developed, implemented and used.MethodsStarting from a literature review, including relevant pilot projects for ICT-based dementia care, we define the major requirements of a system able to overcome the limitations evidenced in the literature, and how this system should be integrated in the socio-technical ecosystem characterizing this disease. From here, we define the DEDICATE architecture of such a system, and the conceptual framework mapping the architecture over the requirements.ResultsWe identified three macro-requirements, namely the need to overcome: deficient technology innovation, deficient service process innovation, and deficient business models innovation. The proposed architecture is a three level architecture in which the center (data layer) includes patients’ and informal caregivers’ preferences, memories, and other personal data relevant to sustain the dementia journey, is connected through a middleware (service layer), which guarantees core IT services and integration, to dedicated applications (application layer) to sustain dementia care (formal support services, FSS), and to existing formal care infrastructures, in order to guarantee care coordination (care coordination services, CCS).ConclusionsThe proposed DEDICATE architecture and framework envisages a feasible means to overcome the present barriers by: (1) developing and integrating technologies that can follow the patient and the caregivers throughout the development of the condition, since the early stages in which the patient is able to build up preferences and memories will be used in the later stages to maximise personalization and thereby improve efficacy and usability (technology innovation); (2) guaranteeing the care coordination between formal and informal caregivers, and giving an active yet supported role to the latter (service innovation); and (3) integrating existing infrastructures and care models to decrease the cost of the overall care pathway, by improving system interoperability (business model innovation).

PurposeThe childbirth process is a complex and vital event that requires careful analysis and improvement. This experience can shape a woman's perspective on motherhood and even affect her mental health. Healthcare providers must prioritize improving the birth experience for women. In this interdisciplinary research, a combination of business process modeling (BPM) and medicine have been used with the aim of realizing an improved delivery experience and increased maternal satisfaction.Design/methodology/approachThe data collection of this study was done by observing 518 childbirth processes and interviewing the chief of labor, chief residents, and midwives in the obstetrics and gynecology department of a hospital in Tehran from October 2022 to February 2023.FindingsThe research has been done in four main stages. The first phase is to model the primary process and sub-processes of normal vaginal delivery (NVD). The second phase is validation using expert confirmation and process mining (PM). The third phase is the analysis of the causes of maternal dissatisfaction in labor. The fourth phase of the heuristics redesigning and improving the process, in which for the first time three new categories have been presented including hospital-based, patient-based, and medical technique-based results show BPM intervention effect can be far-reaching in improving patient care and optimizing operational efficiency.Originality/valueThis study is one of only a few to adopt a process-oriented perspective to show how BPM can be used in clinical processes and has specifically examined an essential clinical process, i.e. childbirth.Highlights Developing business process management (BMP) applications in a medical special process related to childbirth as interdisciplinary research.A combination of qualitative and quantitative techniques contains engineering software and management approaches for a Case study, Implementation of BPM lifecycle in the women's hospital in Iran, Tehran, for a clinical process, which is called, normal vaginal delivery (NVD) process for fetal expulsion normally.Modeling NVD clinical process and sub-process for the first time by BPMN2.0 notations in visual paradigm (VP) software and Validation of the made model with process mining (PM), by Disco process mining software. This was done through event log collection from HIS at the hospital.Improving the childbirth process by redesigning heuristics and Introducing two new categories special for clinical process improvement for the first time.Clinical process improvement heuristics obtained in this research are not consistent with the previous seven categories presented in previous studies such as Marlon Dumas' book. Therefore, we have introduced two new heuristics to redesign clinical processes compatible with medical centers, including hospital-based, patient-based, and medical technique-based.Providing a framework for clinical process modeling and improvement containing steps and tools.

Raziskovalno vprašanje (RV): Kakšne so možnosti za integracijo procesov dolgotrajne oskrbe in kakšne so posledice za oskrbo starejših oseb v domačem okolju? Namen: Raziskati možnosti za izboljšanje kakovosti in uspešnosti dolgotrajne oskrbe v domačem okolju z osredinjenostjo na prejemnika storitev. Cilj je analizirati obstoječe procese teh storitev, ki jih izvajajo formalni izvajalci. Prav tako na osnovi popisa procesov in analize oblikovati predloge za integracijo procesov ter podati hipotetično oceno učinkovitosti prenovljenega procesa. Metoda: Narejen je predlog modela prenovljenega procesa dolgotrajne oskrbe z uporabo eEPC pristopa k modeliranju procesov. Rezultati: Narejen je popis procesov za pomoč na domu in patronažno varstvo. Oblikovan je predlog prenovljenega procesa dolgotrajne oskrbe oseb v domačem okolju, ki vključuje tudi nekatere ključne usmeritve novega zakona na področju dolgotrajne oskrbe. Z integracijo in prenovo procesov se je zmanjšalo število začetnih in zaključnih dogodkov in dogodkov v procesu, število aktivnosti in odločitev. Ocena finančnega učinka integracije procesov oskrbe je narejena hipotetičnem primeru. Stroški oskrbe bi bili nižji skoraj za 20 %, število delovnih ur formalnih izvajalcev pa bi se zmanjšalo za 7 ur na mesec. Organizacija: Z integracijo storitev oskrbe oseb v domačem okolju in prenovo procesov je možno dosegati višjo kakovost storitev, optimizacijo kadrovskih virov in večjo stroškovno učinkovitost. Družba: S sistematičnim proučevanjem procesov dolgotrajne oskrbe, integracije in optimizacije se lahko zagotavlja kakovostna in kontinuirana oskrba, ki podpira samostojnost posameznika pri skrbi zase. To lahko pozitivno vpliva na kakovost življenja prejemnikov oskrbe in laičnih oskrbovalcev. Prenovljen proces lahko pozitivno vpliva na družbene izdatke. Originalnost: Po našem najboljšem vedenju do sedaj ni bil narejen popis procesov v patronažnem varstvu ali v pomoči na domu. Z implementacijo Zakona o dolgotrajni oskrbi v domačem okolju se bo lahko predlog procesa dolgotrajne oskrbe preverjal in smiselno dopolnjeval. Omejitve/nadaljnje raziskovanje: Model še ni preizkušen v praksi. S testiranjem in uporabo se bodo lahko razkrile pomanjkljivosti prenovljenega procesa. Njegovo kontinuirano nadgrajevanje, osnovano tudi na raziskovanju posameznih elementov modela procesa, bo lahko zagotavljalo dolgoročno uspešnost modela procesa. Za to je pomembno nadaljnje proučevanje dejanskih finančnih in drugih učinkov predlaganega modela procesa prek strukturnih kazalnikov učinkovitosti prenove procesa.
