A decrease in blood cholesterol after gastrostomy could impact survival in ALS

Abstract: Although the global benefits of gastrostomy have been proven in amyotrophic lateral sclerosis (ALS), the impact on biological parameters has not been explored yet. The aim of this preliminary work was to evaluate the modification of biological parameters in patients with ALS undergoing gastrostomy. We retrospectively collected clinical and biological data from 44 patients having undergone gastrostomy at three time points (T0, T1 and T2: before, at the time of and after gastrostomy). We examined the relationshi… Show more

“…This is consistent with a proposed metabolic shift towards an increased peripheral use of lipids in ALS and with the evidence of hypolipidemia in pre‐symptomatic murine ALS models . Evidence that the variations of total cholesterol and low‐density lipoprotein cholesterol concentrations after gastrostomy are negatively associated with survival confirms the involvement of lipid homeostasis in the disease. Also, studies in which ANNs were applied to unravel otherwise hidden connections between variables in sporadic ALS highlighted an association between single nucleotide polymorphisms in genes involved in lipid pathways—including the APOA4 gene ‐ and the disease…”

“…Yet, most authors agree that hypercholesterolemia, present as elevated levels of total cholesterol and LDL-c or a higher LDL-c/HDL-c ratio, associates with longer survival. This association, however, did not reach significance after adjusting for potential confounding demographic and clinical factors (37, 40, 43, 4951).…”

“…The link between lipid concentrations and survival remains an enigma in ALS. Nevertheless, a recent study has revealed that higher caloric intake improves survival and that low cholesterol may end up being deleterious in ALS patients ( Blasco et al, 2017 ). A phase III clinical trial evaluating the safety and the tolerance of KD in ALS patients fed through a gastrostomy tube has been conducted in the United States (clinicaltrials.gov NCT01016522), but results have not yet been published.…”
