A decade of living lobar lung transplantation: recipient outcomes

Abstract: These results support the continued use of living lobar lung transplantation in patients deemed unable to await a cadaveric transplantation. We consider patients undergoing retransplantations and intubated patients to be at significantly high risk because of the poor outcomes in these populations.

“…Living-donor lobar lung transplantation (LDLLT) is an alternative to deceased-donor lung transplantation for severely ill patients with end-stage lung disease (1)(2)(3). This procedure has been used widely with good results because of a critical deficit in deceased-donor organs, particularly in Japan (3)(4)(5).…”

“…Perioperative mortality of retransplantation was only 7.7% after LDLLT versus 42.3% after CLT. USC group reported that ventilator dependency and re-transplantation were significant risk factors of death in their 123 LDLLTs (13). In our experience, all 14 patients on a ventilator for as long as 7 months underwent successful LDLLT.…”

“…[173][174][175][176] Living donors have been an important, although decreasing, source of organs for patients requiring kidney, pancreas after kidney, and liver transplants. 100 However, the latest US OPTN/SRTR report records only nine living donor lung transplants in the US since the implementation of the LAS and only two since 2008 through 2012.…”
