A Current Appraisal of Problems with Gangrenous Bowel

Abstract: Gangrenous bowel most often results from hernia, adhesions and mesenteric insufficiency. The overall mortality rate for 151 cases was 37%. This figure was 20% for hernia, 23% for adhesions and 74% for mesenteric insufficiency. In the latter category where bowel resection was feasable the mortality rate was 40%. Other causes of bowel gangrene had a mortality rate of 28%. In many instances the pathophysiologic processes were of such a nature that current medical expertise has not reached a level of development t… Show more

“…Mortality rate of 15-30% has been reported in most series, and this is substantially higher than mortality rate of 5-8% after simple, nonstrangulated obstruction (4)(5)(6)(7)(8). In approximately 30% of cases with strangulation, reversible ischemia may be present (9), and early diagnosis and operation may be vital in such situation.…”

“…Unfortunately, a nonselective, nonoperative plan can be dangerous [5, 6]. When embarking on an initial nonoperative approach, surgeons must be flexible to manage those patients properly with persistence or progression of their symptoms because late recognition of a strangulation obstruction is associated with markedly increased morbidity and mortality [7, 8]. …”

“…Small bowel ischaemia is a potentially fatal complication of small bowel volvulus: mortality in non-infarcted bowel is 5–8%5 and in infarcted bowel 40–100% 6 , 7. Ischaemia is notoriously challenging to diagnose in that bowel enhancement can be normal, increased, decreased or even absent.…”
