A critical threshold for global pediatric surgical workforce density

Abstract: Purpose 1.7 billion children lack access to surgical care, particularly in low- and middle-income countries (LMIC). The pediatric surgical workforce density (PSWD), an indicator of surgical access, correlates with survival of complex pediatric surgical problems. To determine if PSWD also correlates with population-level health outcomes for children, we compared PSWD with pediatric-specific mortality rates and determined the PSWD associated with improved survival. Methods … Show more

“…Middle and low income countries have respectively 9.4 and 5.1 PS/100.000 children 4 . Recent data suggests that there is a correlation between PSWD<0.37 with decreased odds of survival for specific pediatric conditions 1 . Accordingly, our findings reveal that mortality rate for congenital gastrointestinal tract malformations is higher in the lower PSWD regions.…”

“…No country in Africa has reached the LCoGS’s recommendation of 20 specialist surgeons, anaesthesiologists and obstetricians per 100 000 population 1 23. The shortage of paediatric surgeons is concerning, and to aggravate, the existing specialised surgeons are mostly concentrated in major cities 24.…”

“…Trainee engagement with GICS’ diverse offerings and extensive network of peers and mentors provides unparalleled career development in the avenues of clinical care delivery, education, research, and advocacy. The global shortage of pediatric surgical care providers is large and disproportionately affects LMICs, where the median pediatric surgical workforce density is approximate 1% of that in HICs [ 19 ]. While interest in pursuing careers in pediatric surgical care exists, trainees—particularly in low-resource settings—often lack the mentorship, networks, opportunities, and financial support necessary to realize these career goals.…”
