A critical review of trials of transcranial direct current stimulation and trigeminal nerve stimulation for depression: the issue of treatment-emergent mania

Abstract: Safety assessment is of fundamental importance in clinical research. TEM is a very important safety issue in MDD trials. Further and larger controlled trials will help to clarify both the safety and the clinical effects of combinations of pharmacotherapy and tDCS or TNS in daily clinical practice.

“…4 Electroconvulsive therapy offers more long-lasting symptom relief; however, technical challenges and the expense prevent it being as the first-line treatment in daily care. 5 In recent years, there has been growing interest in applying transcranial direct current stimulation (tDCS) for treatment of depression. 6 Results from clinical practices, trials, and some meta-analyses demonstrate favorable potentials for application of tDCS.…”

“…12 Treatment-emergent mania has been reported as a possible side effect of tDCS. 5 In a critical review, 7 trials of tDCS were evaluated with a total of 319 participants. Only 9 (5.38%) participants in active tDCS groups with simultaneous antidepressants therapy showed treatment-emergent mania.…”

“…Trigeminal nerve stimulator (TNS) is the first medical device in ADHD with FDA approval (obtained in 2019) and indicated in children at age 7-12 without ongoing ADHD pharmacotherapy [5]. The use of TNS in neuropsychiatric disorders is not limited to ADHD and includes epilepsy, depression, pain disorders, and migraine [6,7].…”

“…Trigeminal nerve stimulation (TNS) is thought to have neuromodulatory effects on cortical and subcortical regions associated with neuropsychiatric disorders. Studies have shown that TNS may have therapeutic effects on various neuropsychiatric conditions, such as drug‐resistant epilepsy, post‐traumatic distress syndrome, depression, and the negative symptoms of schizophrenia . Thus, hypersensitivity of the trigeminal nerve, which possibly underlies PSS, may also be involved in the emotional regulatory aspects of these neuropsychiatric conditions.…”
