A Controlled Trial of Early Adjunctive Treatment with Corticosteroids forPneumocystis cariniiPneumonia in the Acquired Immunodeficiency Syndrome

Abstract: Early adjunctive treatment with corticosteroids reduces the risks of respiratory failure and death in patients with AIDS and moderate-to-severe pneumocystis pneumonia. Because the adverse effects are few, corticosteroids should be included as part of the initial treatment for persons with AIDS who have moderate-to-severe pneumocystis pneumonia.

“…It is therefore possible that without this confounding factor, concentrations of proinflammatory cytokines in BAL fluid in PCP patients would be even higher. Moreover, our principle conclusion of divergence of local and systemic cytokine profile is supported by similar data in HIV-patients with PCP and a rat model [20,26].…”

“…In the four participating centres patients were given adjunctive corticosteroids when the PO 2 was equal to or less than 8 kPa, a lower threshold than mentioned in the NIH consensus [20].…”

“…Additional investigations indicate that P. carinii cell wall ␤-glucans strongly stimulate neutrophilic lung inflammation (3,11). Nonspecific anti-inflammatory therapies, such as adjunctive corticosteroids, have significantly decreased mortality during severe PC pneumonia (31). Thus, a better understanding of glucan-induced inflammation may provide novel means by which lung inflammation can be selectively modulated during this infection.…”

“…These exuberant host inflammatory responses, although necessary for clearance of infection, further promote lung injury resulting in respiratory failure and excessive morbidity. Therapeutic maneuvers that limit excessive inflammation are beneficial to patients with Pneumocystis pneumonia (45)(46)(47)(48). These observations provide considerable initiative to fully understand the mechanisms and signaling pathways induced by P. carinii cell wall glucans, which trigger inflammatory activation of macrophages (8).…”
