A controlled assessment of an asthma self-management plan involving a budesonide dose regimen. OPTIONS Research Group

Abstract: At the end of the 6 month treatment period, there were no significant differences detected between the DM and the SM groups either from the clinic or diary card data. Both groups demonstrated a significant reduction in the number of sleep-disturbed nights, by 75% in the DM group and 77% in the SM group, at the end of the study. In conclusion, for patients with mild-to-moderate asthma, either a doctor-adjusted dose regimen or a peak flow based self-management plan involving budesonide is equally efficacious. “…Reflux symptoms included heartburn, regurgitation, dysphagia, belching and hoarseness and were graded similarly. This asthma symptom scale has been used in previous studies [27][28][29] . Randomization was done after initial screening and all patients received white capsules containing omeprazole 20 mg (Ome; Dr. Reddy's Laboratories; Hyderabad, India) to be taken orally 1 h before breakfast and 1 h before the evening meal and domperidone (Dom; Dr. Reddy's Laboratories, Hyderabad, India) 10 mg orally 3 times daily (1 h before breakfast, lunch and evening meal) or identical placebo capsules (1:1 double-blind randomization).…”

“…Furthermore, it is unlikely to be undertaken by the patient, even if recommended. An alternative approach is for patients with mild asthma to have a simple regimen in which the dose of inhaled corticosteroid therapy is adjusted by the physician at clinic visits, an approach which may be easier to follow [26].…”

“…The study by AYRES et al [5] in this issue of the Journal provides additional evidence of efficacy of a management plan based on peak flow monitoring and inhaled corticosteroid therapy. Both treatment groups regularly recorded peak flow rates, with their inhaled corticosteroid therapy modified in accordance with changes in peak flow, which was undertaken either by the patient at home, or by the doctor on regular clinic review.…”
