A Congenital Isolated Alar Cleft

Abstract: Alar rim defects are mostly acquired, resulting from burns, traumas, or tumor excision. Sometimes they can accompany craniofacial clefts. However, isolated congenital alar defects are extremely rare. The authors present a case of congenital isolated alar cleft.

“…3 Local flaps must be used for the closure of large defects as better aesthetic results are obtained. 2,4 The Mutaf triangle closure technique, which allows 3-layer repairs, can be considered an ideal method as there is similar skin color and inclusion of the same anatomical location. Using the Mutaf triangle closure technique for skin defects in these 2 patients, the mucosal defects were covered with full thickness skin grafts and conchal grafts were used for cartilage support.…”

“…4,5 These placodes grow as convex thickenings of the surface ectoderm at the end of the fourth week. 5 Then, primitive nasal pits are reshaped by the collapse of the central portions of the placodes. Medial and lateral nasal processes appear around the nasal pits through mesenchymal proliferation.…”

“…Nasal bones, upper lateral cartilages, and the lateral crura of LLC develop from the lateral nasal process. 5 The nasal apex and dorsum derive from the frontonasal prominence. 1 Developmental defects in the lateral nasal processes result in anomalies of the alar region.…”

“…The size of the defect may vary from a simple cleft to a wide defect including the entire ala nasi. 1,2 Here, we present a patient with a wide CIAD in whom a new procedure, namely Mutaf's triangular closure technique, was used for the reconstruction of the alar defect.…”
