A comprehensive approach for evaluating telemedicine-delivered multidisciplinary breast cancer meetings in southern Scotland

Abstract: Multidisciplinary team (MDT) meetings for decisions on cancer management are a cornerstone of UK cancer policy. We have proposed a comprehensive methodology to assess the clinical and economic effectiveness of telemedicine in this setting, which is being tested in a randomized breast cancer trial. Pre- and post-telemedicine assessment includes attitudes to and expectations of telemedicine, based on semistructured interviews. The communication content of videotapes of the MDT meeting is being scored using Borga… Show more

“…Kunkler et al proposed a comprehensive methodology to assess the clinical and economic effectiveness of VC in MDTs [ 27 ]. This methodology was later tested in a randomized breast cancer trial [ 28 ], where 473 MDT patient discussions in two district general hospitals were cluster randomized to the intervention of telemedicine linkage to breast specialists in a cancer center or to the control group of “in-person” meetings.…”

“…This is an issue here in the United States and internationally. 8 The Duke Cancer Network , committed to providing clinical care, education, and research for patients through affiliations with hospitals in relatively rural locations throughout North Carolina, created an Internet-based platform for a multidisciplinary lung cancer tumor conference. The purpose was to enhance the care of patients lung cancer in the community.…”

“…[8][9][10][11][12] Traditional face-to-face meetings are the most common but may be difficult in the community setting, especially in rural areas, where subspecialized providers may not participate in the same practices and the demands of a busy clinical practice limit physician engagement at centralized locations. Telemedicine, with video or Web conferencing, can provide a necessary service for these rural areas.…”

“…Multidisciplinary case discussion can thus be facilitated by VC (Augestad & Lindsetmo, 2009). Kunkler et al proposed a comprehensive methodology to assess the clinical and economical effectiveness of VC in IDTs (Kunkler et al, 2005). This methodology was later tested in a randomized breast cancer trial (Kunkler et al, 2007), where 473 IDT patient discussions in two district general hospitals were cluster randomized to the intervention of telemedicine linkage to breast specialists in a cancer center, or to the control group of "in-person" meetings.…”
