The efficacy of a combination treatment of arthrocentesis and stabilisation splint for patients with bilateral anterior disc displacement without reduction (ADDWoR) and erosive change of the TMJ remains controversial. To evaluate clinical outcomes of patients with ADDWoR and erosive change of the TMJ after performance of unilateral arthrocentesis and stabilisation splint therapy. A retrospective study of 44 patients (37 females, 7 males, mean age of 34 years) with bilateral ADDWoR and erosive change of the TMJ were included in this study. Their clinical outcomes before and after arthrocentesis and stabilisation splint therapy were compared. Evaluation criteria were as follows: (a) Maximal mouth opening (MMO); (b) Right and left maximal lateral movement (RLM, LLM) and maximal protrusive movement (PM); (c) Visual analog scale (VAS) pain score during MMO, RLM, LLM and PM; and (d) VAS pain score during palpation of masticatory muscles. Wilcoxon signed‐rank test, Mc Nemar test and paired t test were used for statistical analysis. Differences in VAS pain score between arthrocentesis and non‐arthrocentesis sites were not statistically significant except MMO and LLM (P < .05) after 6 months. Differences in mean VAS pain scores for all variables between before arthrocentesis and 6 months follow‐up in the arthrocentesis site were statistically significant. (P < .01). Unilateral arthrocentesis on more symptomatic TMJ and subsequent stabilisation splint therapy was highly successful for pain and achievement of normal range of mandibular movements in patients with both ADDWoR and bony change.

The aim of this study is to investigate the relationship between effusion and disc displacement that were detected in MRIs of temporomandibular joint disorder (TMD) patients Material and Methods: MRIs of 60 patients who had pain and/or dysfunction in the TMJ region were evaluated for the presence of effusion. TMC disc positions of 60 MRI results were divided into three groups: normal disc position (control group, n: 20), disc displacement with reduction (R group, n: 20) and disc displacement without reduction (NR group, n: 20). When evaluating the presence of effusion, hyperintense thin lines or absence of effusion was evaluated as no effusion; while signal intensity of more than 2 mm at the anterior-posterior length or superior-inferior height at the joint space was evaluated as the presence of effusion. Results: According to MRI findings, effusion was observed in 2 of the 20 joints (10%) in the control group, 8 of the 20 joints (40%) in the R group and 17 of the 20 joints (85%) in the NR group. When the groups were compared with each other, there was a statistically significant difference between the groups (p<0.05). Conclusion: In TMD, where normal disc position deteriorates to disc displacement, the prevalence of effusion also increases. It is thought that changes in effusion levels can be used as a marker for evaluating the prognosis of TMD.

Purpose: The aim of this study was to measure the masseter muscles activity by electromyogram before and after arthrocentesis in patients with temporomandibular disorders. Materials and methods: Twelve patients with unilateral painful TMJ, diagnosed by RDC/ TMD as group II and indicated for unilateral TMJ arthrocentesis are considered as the study group. In this group, electromyographic analysis was performed before and four weeks following the arthrocentesis. The healthy control group included 6 volunteers without any signs and symptoms of temporomandibular disorders. Electromyographic analysis was performed and compared to that of the study group. Results: Comparison of the electromyographic values of the affected side in the study group before and after arthrocentesis showed that only the amplitude during moderate clenching revealed significant differences after four weeks (p=0.02). Differences in all other parameters were statistically insignificant Conclusions: TMJ arthrocentesis is an effective technique for the treatment of the disc displacements sub-types of TMDs. Following arthrocentesis, definite improvement in clinical results is achieved, but improvement in muscle function is only partial as measured by surface electromyography (SEMG). SEMG is a simple noninvasive technique for monitoring of the treatment outcomes on muscle activity in TMDs. EMG analysis for the diagnosis of TMDs is not specific, because the EMG values before and after the arthrocentesis were within the range of normal values of the control group.
