Knee osteoarthritis (OA) is a highly prevalent disease among the elderly population that results in joint degeneration, leading to reduced mobility and challenging daily living. Patients can choose from various treatment options depending on the stage of joint degeneration. In developing countries such as Pakistan, it is crucial to comprehend the financial burden borne by patients and their families due to this condition's high prevalence. Therefore, this descriptive study aimed to assess the financial burden of knee OA patients seeking healthcare services by analyzing the healthcare costs and expenses incurred by them. A purposive sampling technique was employed to recruit 112 knee OA patients undergoing any treatment modality from public and private hospitals in Lahore. A semistructured questionnaire collected patients' sociodemographic information and overall out-of-pocket health expenditures. Descriptive statistics and the Mann‒Whitney U test were utilized for data analysis. The patients' mean age was 53.69 ± 11.73 years, with 10.26 ± 4.83 years of education. Stage III knee OA was the most common diagnosis (30.36%), followed by stage IV (28.57%), stage II (22.32%), and stage I (18.75%). The direct medical cost was significantly higher (USD 2,590.97) among patients who underwent surgery than among those who opted for alternative treatment procedures (USD 192.10) (p = 0.001). Similarly, the indirect medical cost was significantly higher (USD 371.75) among patients who underwent surgery than among those who chose other treatment options (USD 23.66) (p = 0.001). The overall cost of illness was 6.02 times higher among patients who had surgery (USD 3,050.93) than among those who opted for other treatment modalities (USD 507.19) (p = 0.001). The study’s findings provide insights into the financial burden borne by knee OA patients' families, emphasizing the need for policymakers to intervene. By understanding the costs of illness borne by patients and their families, policymakers can develop targeted interventions to alleviate the financial burden and improve access to healthcare services for knee OA patients.

Objectives: When osteoarthritis (OA) affects the knee, it causes muscle imbalance and physical limitations. This study aimed to determine the impact of knee pain on quadriceps strength, hamstring length, and physical limitations in individuals with bilateral knee OA. Methods: A cross-sectional study at Khyber Teaching Hospital and Northwest General Hospital, Peshawar, included individuals aged 50–65, who met the American College of Rheumatology criteria. Subjects diagnosed with bilateral knee OA (grade II or greater on the Kellgren–Lawrence grading scale) and experiencing pain in both knees (numeric pain rating scale [NPRS] score ≥ 1) were selected through purposive sampling (n = 70). Data collection involved the NPRS, modified belt stabilizer hand-held dynamometer, active knee extension test, and Western Ontario and McMaster Universities Arthritis Index (WOMAC) Urdu version questionnaire. Measurements from both knees assessed quadriceps muscle strength, hamstring length, and physical limitations. Results: The median interquartile range (IQR) of quadricep strength for the more painful knee was 0.53 (0.36) Nm/kg and 1.35 (0.34) Nm/kg for the less painful knee showing a significant difference in quadriceps strength (P < 0.01). In terms of hamstring length, the more painful knee had a median (IQR) of 29.0 (2.0) degrees whereas the less painful knee recorded 11.0 (6.0) degrees indicating a significant difference (P < 0.01). Likewise, there was a significant difference in WOMAC scores between the less painful and more painful knees (P < 0.01). Conclusion: This study found that individuals experiencing more knee pain demonstrated decreased quadriceps muscle strength and increased hamstring muscle tightness. In addition, those with more painful knees exhibited greater physical limitations in bilateral knee OA.

Osteoarthritis (OA) and diabetes mellitus (DM) are two medical conditions that frequently exist together and cause poor consequences. OA is described as joint pain and is one of the main reasons for impairment. Diabetes is high levels of glucose in blood because of disruption in insulin metabolism. Objective: To compare gender variations of severity of pain, stiffness and difficulty in functional performance among patients of second stage knee OA with diabetes. Methods: Data were collected from 360 individuals with mean age 52.32 ± 4.989 years for males and 50.73 ± 4.763 years for females. Individuals with definite osteophytes in knee joint and clinically diagnosed DM were included in the study. The severity of stiffness and functional limitation was calculated using WOMAC questionnaire. Results: The mean of pain score was 9.0833 ± 3.04322, stiffness score was 4.3389 ± 1.12446 and the mean of physical function score was 34.0944 ± 8.65135. Overall, 109 patients had less severe symptoms while 71 had more severe symptoms. Among 180 female participants, n=15 had mild, n=139 had moderate and n=26 had severe pain on NPRS. The mean of the pain score was 9.8778 ± 2.90762. The mean of stiffness score was 4.5167 ± 1.28365 and the mean of physical function score was 37.1611 ± 7.98963. Conclusion: Our findings concluded that the gender difference of severity of pain and physical function limitation was statistically significant, but no significant difference was found in severity of stiffness. Also, females with diabetes had more severe symptoms than males with diabetes
