A comparison of covered vs bare expandable stents for the treatment of aortoiliac occlusive disease

Abstract: COBEST demonstrates covered and bare-metal stents produce similar and acceptable results for TASC B lesions. However, covered stents perform better for TASC C and D lesions than bare stents in longer-term patency and clinical outcome.

“…Сердечно-сосудистые заболевания / Cardiovascular medicine "kissing" в сочетании с новыми модификациями стентов и баллоных катетеров свело к минимуму частоту интра-и послеоперационных осложнений [15][16][17]. Согласно проводимому мета-анализу, в ко-тором оценивались результаты эндоваскулярной ре-конструкции с 2000 по 2010 гг., сообщается о 5-лет-ней первичной проходимости 65% стентов.…”

“…The Covered Versus Balloon-Expandable Stent Trial (COBEST) observe a higher patency rate with covered stent in comparison to non-covered stents in aortoiliac disease; however, non-covered stent group show unusually high restenosis rate. [96] Balloon-expandable stents are mostly used to revascularize IC and isolated occlusive aortoiliac disease. "Self-Expanding Stents" are mostly made of Nitinol, it possess thermal shape memory and are more resilient to mechanical stresses by expanding on deployment at body temperature and then re-expanding after external radial compression.…”

“…15) Covered iliac stents have recently suggested improvement in the percutaneous management of complex aortoiliac bifurcation disease. 16,17) In a randomized controlled trial (RCT) of polytetrafluoroethylene (PTFE) covered stents compared to bare metal stents, there were statistically reduced restenosis rates with covered stents, particularly in the more complex TASC C-D aortoiliac lesions. 18) It should be noted that given the advances in percutaneous treatment of both abdominal and thoracic aortic aneurysms and aortic and mitral valve replacement, iliac artery stenosis may require treatment, even in asymptomatic patients, to facilitate passage of large bore catheters from the femoral artery to the target lesion.…”
