A Comparison of Compression Ultrasound With Color Doppler Ultrasound for the Diagnosis of Symptomless Postoperative Deep Vein Thrombosis

Abstract: Color Doppler ultrasonography has a moderate to low accuracy for the detection of DVT in patients who have had hip and knee replacement surgery. Color Doppler ultrasonography does not increase the detection rate for asymptomatic DVT over compression ultrasound and thus cannot be recommended as a screening test in this setting.

“…Among patients with confirmed PEs, 15 to 50% presented with a DVT that was diagnosed by CDUS. The sensitivity decreases in asymptomatic patients 4,5 and is weak for the detection of pelvic DVT (from the external iliac vein to the inferior vena cava). Contrast venography, a more invasive test, detected DVT in 70 to 80% of patients with confirmed PEs.…”

“…In contrast to symptomatic patients, US procedures are not sufficient to screen patients without symptoms (e. g., in postoperative situations such as in hip surgery) [62,68]. It is assumed that the lower sensitivity in this group of patients is given by the higher percentage of isolated calf-vein thrombosis.…”

“…These figures are remarkably higher than those reported in studies using noninvasive methods to detect DVT. Sensitivity of real-time B-mode ultrasonography, alone or combined with Doppler, to overall postoperative DVT in major orthopedic surgery ranges from 47% to 55% (13,18,19). Surprisingly, results are not improved with color-Doppler techniques (19)(20)(21).…”

“…Sensitivity of real-time B-mode ultrasonography, alone or combined with Doppler, to overall postoperative DVT in major orthopedic surgery ranges from 47% to 55% (13,18,19). Surprisingly, results are not improved with color-Doppler techniques (19)(20)(21). A recent meta-analysis of studies assessing the accuracy of ultrasound screening for DVT in patients after orthopedic surgery has confirmed the disappointing sensitivity of venous ultrasound imaging for postoperative DVT, as shown by contrast venography (22).…”
