A Comparison of Complications in 400 Patients After Native Nail Versus Silicone Nail Splints for Fingernail Splinting After Injuries

Abstract: It seems to be advantageous to use the native nail for splinting after trauma, when possible. In case of a destroyed and unusable nail plate, a nail substitute has to be used.

“…Another important aspect is the appearance of synechia between the dorsal skin fold and the nail bed contributes to anomalous forms of the nail plate [2,27]. The insertion of BC dressing between the ventral surface of the DUP and the nail bed prevented the formation of synechiae and served to guide the growth of the new nail plate.…”

“…It exerts protection of the distal phalanx against traumatic impacts, it helps in the pincer function of small objects, it collaborates with fine sensitivity and contributes to the appearance of hands and feet. Esthetically unpleasant nails and symptoms associated with poor functioning of the nail apparatus cause painful symptoms and reflect on individual work capacity with socioeconomic impact [2].…”

“…Although the rates of healing in the three groups were not statistically different; however, they found that replacement of the native NP had significant advantages on adherence and pain compared with the other dressings. Weinand et al 6 compared 240 native NP and 161 silicone NP splints for fingernail splinting after fingertip injuries. In the native NP splint group, 63 of 240 patients (26%) showed nail deformities.…”

“…A few studies have compared the use of a native NP and an artificial NP substitute in fingertip injuries; however, no consensus currently exists regarding the most appropriate treatment modality. 3 , 5 , 6 , 7 Draelos 8 indicated that a prefabricated transparent nail-shaped plastic plate called artificial NP or fake NP could be an appropriate NP-substitute device.…”

“…Of the four fingers treated with glue in the ER, one required revision due to early secondary infection. In a large comparative study, Weinand et al [22] found a higher number of nail dystrophies and infections when an artificial nail was used instead of the original nail. We cannot address this risk since few of our patients received an artificial nail.…”
