A Comparison Between the Bonfils Intubation Fiberscope and McCoy Laryngoscope for Tracheal Intubation in Patients with a Simulated Difficult Airway

Abstract: In the hands of trained operators, there appears to be no clinically significant difference in success, time to intubation, or adverse outcomes, when comparing the Bonfils with the McCoy laryngoscope, in the setting of a simulated difficult airway. The choice to use either device should remain based on appropriate patient selection, available aids, individual operator's experience, and economic circumstances.

“…Therefore the interincisor distance, sternomental distance, thyromental distance, and modified Mallampati tests are all predictor of a difficult airway [ 9 ]. At this point, several studies have been performed by applying a cervical collar for the limitation of cervical movement and mouth opening [ 5 , 10 , 11 , 12 , 13 ]. The results of our study showed a decreased interincisor distance and range of neck movement after application of the cervical collar.…”

“…Inspection of the forest plot of 4 available studies [21–23,25] that recruited a total of 297 patients (video-stylet group, n = 149 vs laryngoscope group, n = 148) revealed a comparable intubation time between the video-stylet and the laryngoscope groups (MD = 4.53, 95% CI: –8.45 to 17.51, P = .49; I 2 = 85%) (Fig. 5).…”

“…35 However, our group of consultant anaesthetists have extensive experience in using the Bonfils fibrescope in difficult airway patients under anaesthesia, and there are increasing data to support its use in difficult airways. 3537 The Bonfils fibrescope has advantages over other forms of indirect laryngoscopy in patients with peri-glottic lesions due to its increased manoeuvrability around these lesions, and its rigid stylet component can push lesions aside and can also act as a guide for tracheal tube rail-roading and subsequent insertion through a distorted peri-glottis. 32,38 As far as we are aware, this is the largest reported case series on the use of the Bonfils fibrescope in H&N patients.…”
